血液灌流联合连续性肾脏替代治疗技术治疗重症胰腺炎

目的观察血液灌流联合连续性肾脏替代治疗（continuous renal replacement therapy,CRRT）技术治疗重症胰腺炎（severe acute pancreatitis,SAP）患者的临床疗效,并探讨其机制。方法将30例患者随机分成两组：联合治疗组15例采用CRRT＋血液灌流治疗;对照组15例采用单纯CRRT治疗。比较两组治疗前后临床症状、APACHEⅡ评分改善、TNF-α及IL-6变化情况。结果两组治疗后,患者临床症状缓解,血清TNF-α及IL-6均降低,但联合治疗组明显优于对照组（P〈0.05）。结论血液灌流联合CRRT治疗重症胰腺炎可有效调整炎症因子水平,提高患者的治疗效果。

Analysis of Hemoperfusion Combined with Continuous Renal Replacement Therapy in the Treatment of Patients with Severe Acute Pancreatitis

Objective To observe clinical efficacy of hemoperfusion with CRRT in treating patients with severe pancreatits（SAP） and discuss the mechanism.Methods 30 patients were divided into 2 groups randomly,among which 15 were treated with hemoperfusion combined with CRRT,and the other 15 patients in control group were treated only with CRRT.Compare the clinical symptoms,the change of APACHEⅡscore and the level of TNF-α and IL-6 before and after treatment.Results Clinical symptoms of the patients were improred after the 2 groups were treated,and the level of TNF-α and IL-6 were both reduced,however,the efficacy in hemoperfusion group was better than that in control group（P0.05）.Conclusion Hemoperfusion combined with CRRT was effective in changing the level of inflammatory and improving patients＇treatment effect.
