主动脉内球囊反搏在危重心脏外科围术期的应用(附33例临床分析)

摘    要:目的总结主动脉内球囊反搏(intra-aortic balloon counter-pulsation,IABP)在心脏外科围术期抢救危重患者的效果。方法2004年6月至2007年5月33例围术期危重心脏病患者实施了IABP,其中冠心病26例,瓣膜病7例,均在常规低温体外循环、心脏停跳下行冠脉搭桥或瓣膜手术。术中植入IABP 22例,术后植入11例。结果死亡10例,总成功率69.7%,其中冠心病成功率81.8%,瓣膜病成功率28.6%,无一例出现与IABP相关的肢体缺血坏死等严重并发症。结论IABP是一种安全、有效的心脏外科围术期辅助循环支持手段。

Abstract:Objective To summarize the effect of the intra-aortic balloon counterpulsation(IABP) during the perioperation of cardiac surgery.Methods From June 2004 to May 2007,33 cases underwent cardiac surgery were performed IABP under the routine hypothermic cardiopulmonary bypass and cardiac arrest,including 26 of the coronary artery bypass grafting(CABG) and 7 of valve replacement.22 cases were performed IABP during the operation,the others after operation.Results Ten cases died.The total success rate of IABP was 69.7%,with 81.8% in CABG and 28.6% in valve replacement.No severe complication as ischemia or necrosis occurred in all cases.Conclusion IABP is a safe and effective mechanical assist device during cardiac operative procedures.
