肺癌患者外周血骨桥蛋白检测的临床意义

目的探讨血清骨桥蛋白检测在肺癌患者临床诊疗中的应用价值。方法选择不同临床TNM分期的肺癌患者120例作为肺癌组,同期肺炎性假瘤患者23例作为疾病对照组,49例门诊体检正常者作为健康对照组,应用酶联免疫吸附法(ELISA)进行血清骨桥蛋白水平检测。同时对其中67例临床治疗有效的肺癌患者行治疗前后(放化疗或手术前后)该肿瘤标志物血清水平比较。结果血清骨桥蛋白水平在Ⅰ~Ⅱ期患者中为(73.7±32.9)μg/L,在Ⅲ期患者中为(144.6±44.7)μg/L,在Ⅳ期患者中则可达(395.8±105.2)μg/L,不同TMN分期患者间血清骨桥蛋白水平存在显著性差异(P均<0.05)。67例治疗有效者血清平均骨桥蛋白水平由治疗前的(61.27±18.38)μg/L降至治疗后的(32.06±11.53)μg/L,治疗前后存在显著性差异(P<0.05)。结论随着肺癌的转移以及恶性程度增高,人骨桥蛋白血清水平逐步升高,经治疗缓解的肺癌患者血清中骨桥蛋白水平下降明显,说明骨桥蛋白血清水平是一个较好的评价肺癌进展程度的血清学指标,且有助于临床对肺癌临床疗效的评判。

The clinical significance of detection of serum osteopontin in patien with lung cancer

Objective It is to approach the clinical significance of detection of serum osteopontin level in the treatment and diagnosis of lung cancer.Methods The levels of serum osteopontin in 120 patients with lung cancer in different TNM stages,23 pulmonary inflammatory pseudotumor(PIP) as disease controls and 49 out-patients physical examination normal subjects as healthy controls were measured by enzyme linked immunosorbent assay(ELISA),and further comparison of serum osteopontin level before and after treatment in 67 patients who have clinical efficacy after received therapy(chemotherapy or operation).Results In TNM stage Ⅰ and Ⅱ patients,the mean value of serum osteopontin level was(73.7±32.9) μg/L;in TNM stage Ⅲ patients,the mean value of serum osteopontin level was(144.6±44.7) μg/L;in TNM stage Ⅳ patients,the mean value of serum osteopontin level was(395.8±105.2) μg/L,there was significant difference between the different group(P<0.05).After treatment,the level of serum osteopontin was decreased significantly from(141.6±59.7) μg/L to(43.9±16.5) μg/L in 67 patients who have clinical efficacy after received therapy,there was significant difference between the before and after therapy(P<0.05).Conclusion The serum osteopontin level can rise as the increase of TNM stages of the lung cancer,and the serum osteopontin level can significantly decrease when the treatment be available.These suggest that osteopontin may be a serology maker to assess the progression of lung cancer,and help to evaluate the efficacy of clinical treatment.
