抗核抗体不同检测方法在自身免疫性肝炎中应用及对比分析

目的:评价以HEp-2细胞为底物的间接免疫荧光法(IFA)和包被多种纯化核抗原的酶联免疫吸附试验(ELISA)检测抗核抗体(ANA)的特点及其在自身免疫性肝炎(AIH)诊断中的应用价值。方法:同时采用IFA法和ELISA法检测疑似AIH患者血清ANA共110例,阳性率分别为IFA法62.7%,ELISA64.5%.其中IFA法检测结果以斑点型和均质型多见。将两种方法检测结果的差异进行统计学分析。结果:两种方法检测结果差异无统计学意义。结论:IFA作为ANA检测的推荐方法具有直观、可分型等特点,且能提供与自身免疫性疾病相关的核型信息,但需要荧光显微镜及具有较丰富判断经验的技术人员;而ELISA法操作简单,无需特殊仪器,并可通过测定值来评价患者体内抗体的浓度。

Application and Comparision of Different Detection Techniques of Antinuclear Antibodies in Autoimmune Hepatitis

Objective:To investigate the characteristics and applications of indirect immunofluorescence(IFA) and enzyme-linked immunosorbentassay(ELISA) for detecting antinuclear antibodies(ANA) in diagnosis of autoimmune hepatitis(AIH).Methods:The serum ANA in 110 cases of doubtful AIH were detected by both IFA and ELISA methods.The positive rate of IFA and ELISA was respectively 62.7% and 64.5%.The results of IFA methods mainly showed speckled pattern and homogeneous pattern.The results of two types of detection methods were statistically analyzed.Results:The results of two detection methods had no statistical difference.Condusiom:The recommended IFA technique for detecting ANA was direct and easily typing one,which provided relevant informations of typing about autoimmune diseases.The IFA technique needed practiced technicians,but ELISA was simple without special instruments and its results could evaluate the concentration of serum antibodies.
