腰麻联合硬膜外麻醉与单纯硬膜外麻醉在合并心脏病产妇剖宫产术中麻醉效果的观察比较

目的探讨腰麻联合硬膜外麻醉（CSEA）在合并心脏病产妇剖宫产术麻醉中的应用效果。方法选取江门市第二人民医院自2008年4月至2012年4月收治的合并心脏病的产妇38例，应用随机数字表法将符合本研究纳入标准和排除标准的患者分为两组，每组19例。其中观察组产妇给予腰麻联合硬膜外麻醉，对照组产妇给予硬膜外麻醉（EA）。观察两组产妇的麻醉临床效果。结果观察组产妇麻醉最高阻滞平面、到达最高阻滞平面时间、I—D时间均低于对照组，观察组产妇麻醉效果优良率较对照组高，差异有统计学意义（P〈0．05）。结论腰麻联合硬膜外麻醉具有良好的麻醉阻滞效果，值得在临床上广泛推广。

Comparative observation of lumbar anethesia combined with epidural anesthsia and single epidural anesthesia in patients with heart disease in maternal cesarean section

Objective To investigate the effect of combined spinal and epidural anesthesia （CSEA） application in anesthesia on heart disease women during cesarean section. Methods From April 2008 to April 2012 in our hospital, 38 maternity patients with heart disease were chose as object of study. The method of random digits table was used for patients who met the inclusion criteria and exclusion criteria in this study, they were divided into two groups, with 19 cases in each group. The patients in ob- servation group were given combined spinal and epidural anesthesia, the patients in control group were given epidural anesthesia （EA）. The anesthetic effects of the two groups were observed. Results The time reaching the highest plane of anesthesia block, the highest plane of anesthesia block and I-D time in the observation group were lower than those in the control group. The excellent and good rate of anesthesia effect in the observation group was higher than that in the control group, there were signifi- cant differences （P 〈 0. 05 ）. Conclusion Combined spinal and epidural anesthesia has good anesthesia effect, and is worthy of popularization in clinics.
