摘    要:目的:评价国内重要文献数据库的资料中损伤控制应用于急诊外科多发伤临床疗效。方法:计算机检索中国学术期刊全文数据库(CNKI)、中国生物医学文献数据库(CBMdise)、Pubmed(截止至2013年1月)及其参考文献,语种为中文。筛选所有损伤控制应用急诊外科多发伤的随机或半随机对照试验(ROT),对照组为采用常规治疗的急诊外科多发伤患者。检索时限定研究对象为急诊外科多发伤患者,由两名评价员独立检索和评价质量,采用RevMan5.0软件进行Meta分析。结果:共检出符合标准的应用损伤控制治疗急诊外科多发伤的RCT文献38篇,筛选出采用ISS评分并行临床疗效判定的6篇文献(损伤控制治疗组为226例,对照组为239例),与对照组比较,应用损伤控制治疗急诊外科多发伤疗效显著,两组间差异有统计学意义。结论:中国目前数据库的临床资料显示,应用损伤控制治疗急诊外科严重多发伤能降低病死率。

Abstract:Objective:To assess the clinical effect of damage control surgery on patients with severe multiple trau- ma according to domestic medical literatures. Methods:China National Knowledge Infrastructure (CNKI), the China Biological Medicine Data-base (CBMdisc), Pubmed (all till Jua.2013) and the reference lists of related articles were searched. Two appraisers assessed the quality of studies and data independently. The RevMan 5.0 was used for staffs- tical analysis.Results:Thirty-eight randomized controlled trials about treating severe multiple trauma by con- trol surgery complying with the standard were identified and six randomized control tests were selected by ISS(injury severity score standard) and mortality rate, which included 226 cases in treatment group and 239 cases in control group. Meta-analysis showed mortality rate in treatment group showed significant difference compared with that in control group. Conclusion:The Meta - analysis results from databases indicate that damage control surgery can provide effective improvements for mortality in severe multiple trauma in emergency surgery.
