中西医结合三步疗法对272例儿童哮喘疗效观察

目的:观察三步疗法对控制儿童哮喘乃至临床治愈的治本效果.方法:对哮喘儿童272例随机分入三步疗法组(三步组)和哮喘菌苗组(菌苗组),分别给予三步计划和哮喘菌苗治疗6个月.对两组的近期疗效和哮喘控制者随访1～3年,进行对照观察.结果:疗程末三步组临床有效率(95.3%)高于菌苗组(79.9%)(P<0.05);三步组的哮喘年控制率分别为98.2%、94.6%,临床治愈率(88.3%)均明显高于菌苗组(78.0%、74.7%、56.1%)(P<0.01).治疗中三步组的副作用亦轻于菌苗组.结论:中西医结合三步疗法对儿童哮喘有较好疗效,其作用机制可能与抑制IgE的合成,减少嗜酸性细胞(EOS)及炎性介质的生成有关.采用平喘、脱敏来改善气道的过敏性炎症的高反应性疗效较佳;中西医结合疗法有效地阻抑哮喘复发,使缓解期不断延长,甚至不再发病.提示中西医结合三步疗法在促进儿童哮喘的临床治愈上,有一定的治本意义.

Clinical Observation on Treating 272 Cases of Children Asthma by Three Steps Therapy of Integrated Traditional Chinese and Western Medicine

Objective: To investigate the effect on controlling and clinical curing children asthma by Three Steps Therapy(TST), which including controlling children syndrome, improve sensitive character and depress asthma reoccurring. Methods:272 cases of asthma children were randomly divided into TST group and vaccae group, both being treated for 6 months. Those syndrome controlled in both group were keeping observed and compared for 1 to 3 years. Results: The clinical effective rate(95.3%) in TST group was obviously higher than that(79.9%) in vaccae group(P<0.05); the syndrome controlling rate per year(98.2%, 94.6%) and clinical curing rate(88.3%) in TST group were evidently higher than those in vaccae group(78.0%, 74.7%, 56.1%)(P<0.05); the side effect in TST group was lower than that in vaccae group. Conclusion:TST of integrated traditional Chinese and western medicine has better curative effect on children asthma, which mechanism is some related to depress IgE, reducing EOS and inflammation interferes, increase the high allergic inflammation of airway by controlling asthma and relieving sensitivity. Treating with integrated traditional Chinese and western medicine can effectively depress asthma reoccurring and lengthen the reliving period continually, even to no recurrence longer. It indicates that TST of integrated traditional Chinese and western medicine has some essential therapy effect on curing children asthma.
