Gamma钉及解剖钢板内固定治疗股骨粗隆间骨折疗效比较

目的比较Gamma钉内固定及解剖钢板内固定治疗股骨粗隆间骨折的临床疗效。方法82例股骨粗隆间骨折，Gamma钉组52例，解剖铜板固定30例，比较两种方法的手术时间、术中失血量等疗效指标和术后并发症的发生及术后髋关节功能的恢复。结果Gamma钉组术中出血量及手术时间少于解剖钢板组（P〈0．05），并发症及手术优良率差异无统计学意义（P〉0．05）。结论与解剖钢板相比，Gamma钉内固定具有手术时间短，术中出血少等优点。

Comparison of efficacy between Gamma nail and anatomic plate inner fixation in treatment of intertrochanteric fracture

Objective To compare the clinical effect between Gamma nail and anatomic plate inner fixation in treatment of intertrochanteric fracture. Methods Eighty two patients with intertrochanteric fracture were included in the study, among them the fracture was fixed with Gamma nail in 52 cases and fixed with anatomic plate in 30 cases. The operating time, the volume of intraoperative blood loss and postoperative complications were compared between two groups. Results The operating time was shorter and blood loss was less in Gamma nail group than those in anatomic plate group （P 〈 0. 05 ）. Coxa varo occurred in 4 cases in Gamma nail group and 2 cases in anatomic plate group; pulmonary infection occurred in 2 cases in anatomic plate group. There was no statistical difference in rate of postoperative complications between two groups （P 〉 0. 05 ）. Long-term postoperative follow-up indicated that the excellent and good rate of hip functional recovery was 94. 2% in Gamma nail group and 93.4% in anatomic plate group （P 〉 0. 05 ）. Conclusion Gamma nail has advantages over anatomic plate in terms of less blood loss and shorter operating time and should be the first choice for intertrochanteric fracture.
