丁卡因表麻对全麻诱导气管插管血流动力学的影响

摘    要:目的探讨丁卡因气管黏膜表麻对全麻插管血流动力学的影响。方法将120例ASAⅠ~Ⅱ级普外科择期手术患者随机分为两组,每组60例。试验组:1%丁卡因5 ml采用气管内喷入;对照组:无表面麻醉。诱导方法:咪达唑仑0.05 mg/kg,芬太尼3μg/kg、丙泊酚2 mg/kg、赛机宁0.3 mg/kg。观察指标:记录平均动脉压、心率在插管前、插管时、插管后1、3 min的变化。结果试验组在不同时点与对照组比较都有不同程度减少循环波动的保护作用,差异有统计学意义(P<0.05)。结论全麻诱导时丁卡因表麻可减轻气管插管心血管反应。

Abstract:Objective To observe the effect of tracheal mucosal surface anesthesia with dicaine on intubation cardiovascular response.Methods 120 cases of patients(ASAⅠ-Ⅱ)undergoing selective general surgery operations were randomly divided into 2 groups with 60 patients in each group.Experiment group: 1% dicaine was sprayed intratracheally before intubation;Control group:there was no surface anesthesia.Tracheal intubation was performed with midazolam 0.05mg/kg,fentanyl 3μg/kg,propofol 2mg/kg and cisatracurium 0.3mg/kg..Mean arterial pressure(MAP) and heart rate(HR) were measured after the anesthesia induction,at the beginning,1minute and 3 minutes after tracheal intubation.Results There were much difference degrees on protection effection against decreasingblood pressure between experiment group and control group(P<0.05).During the intubation,the HR、ABP were stable in experiment group.Conclusion Surface anesthesia with dicaine during general anesthesia induction can reduce the cardiovascular response of tracheal intubation
