老年高血压病患者血清MCP-1水平与胰岛素抵抗的相关性

摘    要:目的探讨老年高血压患者血清单核细胞趋化蛋白-1(MCP-1)水平与胰岛素抵抗(IR)的关系。方法将60例老年高血压非糖尿病患者按胰岛素抵抗指数分为高血压组(n=29)和高血压伴胰岛素抵抗(IR)组(n=31),以20例老年健康者作为对照组,放射免疫法测定血清胰岛素,以稳态模型评估法(HOMA)评价胰岛素抵抗(HOMA-IR)和胰岛β细胞功能指数(HOMA-islet),采用Elisa法测定血清MCP-1水平。结果高血压组的MCP-1水平高于健康对照组(P〈0.05),高血压伴IR组的血清胰岛素、MCP-1、HOMA-IR、HOMA-islet水平均高于高血压组和健康对照组(P〈0.05)。相关性分析显示高血压病患者血清MCP-1水平与FPG、TC、LDL-C、血清胰岛素、HOMA-IR、HOMA-islet呈明显正相关(P〈0.05)。结论老年高血压伴胰岛素抵抗患者血清胰岛素水平、HOMA-IR、HOMA—islet、血清MCP-1水平较健康老年人和老年高血压无胰岛素抵抗患者均明显增高,存在高胰岛素血症及代偿性胰岛p细胞功能增高;老年高血压病患者血清MCP-1水平与FPG、TC、LDL-C、血清胰岛素、HOMA—IR、HOMA-islet呈明显正相关。

Abstract:Objective To investigate the relationship between serum level of MCP-1 and insulin resistance in elderly patients with hypertension. Methods 60 cases of elderly patients with hypertension were divided into hypertension group( n = 29)and hypertension with insulin resistance group( n = 31 )according to insulin resistance index, and 20 healthy individuals were selected as the control goups. Serum insulin was measured by radioimmunoassay, to evaluate insulin resistance and islet βell function index by homeostasis model assessment method, and serum level of MCP-1 was measured by Elisa method. Results The serum level of MCP-1 in hypertension group was higher than that in control group( P 〈 0.05). The levels of serum insulin, MCP-1 ,HOMA-IR,HOMA-islet in hypertension with insulin resistance group were higher than those in hypertension group and control group( P 〈 0.05). Dependability analysis showed serum level of MCP-1 had positive correlation to levels of FPG,TC,LDL-C,serum insulin,HOMA-IR,HOMA-islet. Conclusion The levels of serum insulin and MCP-1 ,HOMA-IR,HOMA-islet were higher than those in hypertension group and control group. Presenting hyperinsulinemia, compensative function enhancement of βell of islet. The serum level of MCP- 1 had positive correlation to levels of FPG,TC,LDL-C,serum insulin,HOMA-IR,HOMA-islet.
