无骨折脱位型颈脊髓损伤的诊治

目的探讨无骨折脱位型颈脊髓损伤的诊断与治疗特点。方法对21例患者行颈椎X线平片、CT、MRI检查,颈部制动,并给予脱水剂、激素、神经营养药物运用,结合影像学资料酌情选择手术及具体术式。结果 21例患者中18例在经治疗后脊髓功能均获得不同程度的改善,手术时机、方式的选择不同,显示不同的治疗效果。结论无骨折脱位型颈髓损伤有其特定的受伤机制,MRI对于该类损伤的诊断具有优越性。应根据脊髓损伤表现及变化和MRI显示脊髓的病理状况选择不同的治疗方法,对于有脊髓压迫的病例手术时机最好在1周内。

Diagnosis and treatmenton without fracture dislocation type cervical spinal cord injury

Objective To discuss the characteristics of the diagnosis and treatment of cerrical spinal cord injury without fracture dislocation.Methods Twenty-one patients underwent cerical X-ray,CT,MRI,neck braking,and were given the dehydrating agents、 hormones、neurotrophic drug use,combining the imaging data to choose the specific procedures.Results 21 patients 18 cases in the treatment of spinal cord function after all different degrees of improvement,operation time,the choice of the ways of different,show different treatment effect.Conclusion Without fracture dislocation type cervical spinal cord injury has its particular injury mechanism for such damage,magnetic resonance imaging(MRI) has superiority.According to the spinal cord injury and change and MRI showed spinal pathological condition choose different treatment methods,for cases of spinal cord compression operation time is best in one week.
