探讨电视胸腔镜下行食管癌切除胸部淋巴结清扫术的效果

目的 对电视胸腔镜下行食管癌切除胸部淋巴结清扫术的效果及安全性进行分析探讨. 方法 整群选取该院2011年1月—2012年5月收治的82例食管癌患者为研究对象,随机分为对照组与观察组,对照组41例患者行传统开胸手术,观察组41患者行电视胸腔镜下食管癌切除术,对比两组患者的治疗效果及并发症发生率. 结果 观察组的术中出血量明显短于对照组,手术时间及住院时间明显短于对照组,住院费用明显高于对照组,差异有统计学意义(P<0.05).观察组的并发症发生率明显低于对照组,差异有统计学意义(P<0.05).两组患者的围术期死亡率与3年生存率比较差异无统计学意义(P>0.05). 结论 电视胸腔镜下食管癌切除淋巴清扫术可有效缩短手术时间,减少术中出血量,提高治疗效果,值得临床推广应用.

Effect of Resection of Esophageal Cancer and Dissection of Lymph Node in Chest with Video-assisted Thoracoscope

Objective To analyze and investigate the effect and safety of resection of esophageal cancer and dissection of lymph node in chest with video-assisted thoracoscope. Methods 82 cases with esophageal cancer admitted in our hospital from January 2011 to May 2012 were randomly divided into the control group and the observation group with 41 cases in each. Patients in the control group were treated by the traditional open thoracic operation, while those in the observation group were treated by the resection of esophageal cancer with video-assisted thoracoscope. And the therapeutic effect and incidence of complications were compared between the two groups. Results Compared with the control group, the observa-tion group had much less intraoperative blood loss, obviously shorter operation time and hospitalization time and much more hospitalization cost, the differences were statistically significant, P<0.05. The incidence of complications was much lower in the observation group than in the control group, the difference was statistically significant, P<0.05. There were no statistical-ly significant differences in the perioperative mortality and 3-year survival rate between the two groups, P>0.05. Conclu-sion Resection of esophageal cancer and dissection of lymph node in chest with video-assisted thoracoscope can effectively shorten the duration of procedure, reduce the intraoperative blood loss, and improve the treatment effect, which is worthy of clinical popularization and application.
