两孔腹腔镜结合曲安奈德治疗小儿腹股沟疝合并鞘膜积液43例临床分析

目的 探讨两孔微型腹腔镜结合穿刺注射曲安奈德治疗小儿腹股沟疝合并精索鞘膜积液手术方法及临床效果.方法 整群收集该院2011年5月—2015年5月诊治的43例小儿腹股沟疝合并鞘膜积液患儿资料进行分析,将其设置为腹腔镜组,选取同期治疗的40例患者作为传统开放组,传统开放组采用传统开放手术治疗,腹腔镜组采用两孔微型腹腔镜结合穿刺注射曲安奈德治疗,比较两组疗效. 结果 腹腔镜组手术时间为(22±2)min,术中出血量为(0±0)mL,住院时间为(3.5±1.9)d,进食及下床活动时间为(7±2)h,显著少于传统开放组手术时间为(52±4)min,术中出血量为(13±3)mL,住院时间为(5.1±2.4)d,进食及下床活动时间为(15±5)h(P<0.05). 结论 两孔微型腹腔镜联合曲安奈德治疗小儿腹股沟斜疝合并精索鞘膜积液手术简单、快捷、安全、有效、美容效果好.

Clinical Analysis of Two-trocar Laparoscope Combined with Triamcinolone Acetonide in Treatment of 43 Cases with Inguinal Hernia in Children with Hydrocele

Objective To discuss the operation method and clinical effect of Two-trocar mini laparoscopy combined with puncture injection of triamcinolone acetonide in treatment of Inguinal hernia in children with funicular hydrocele. Methods The materials of 43 cases from May 2011 to May 2015 in our hospital with inguinal hernia in children with hydrocele were analyzed, and they were regarded as laparoscope group and were given two-trocar mini laparoscopy combined with puncture injection of triamcinolone acetonide treatment, 40 cases treated in the same period were regarded as the traditional open group and were given traditional open surgery, the curative effects of the two groups were compared. Results The operation time, intraoperative blood loss, length of stay and eating and getting out of bed activity time in the laparoscope group were obviously less than those in the traditional open group, (22±2)min, (0±0)mL, (3.5±1.9)d and (7±2)h vs (52±4)min, (13± 3)mL, (5.1±2.4)d and (15±5)h (P<0.05). Conclusion The operation of two-trocar mini laparoscopy combined with triam-cinolone acetonide in treatment of inguinal hernia in children with funicular hydrocele is easy, rapid, safe and effective and has better cosmetic results.
