射频消融离体子宫肌瘤的实验研究

摘    要:目的探讨冷循环射频消融离体子宫肌瘤的效果和安全性.方法对30例单发或多发的离体子宫肌瘤施行射频消融固化治疗,根据瘤体的大小对射频的时间控制、射频次数、表面温度进行了比较研究.结果射频区剖面所见圆形或椭圆形灰白消融面,质地变硬,边缘与肌组织有明显的界线,浆膜与瘤体分离.针尖距肌瘤表面>1.5cm,则表面无变化;如<1.0cm,则瘤体包膜肿胀,表面有脱水现象,进而表面棕褐色,时间较长者,肿瘤可发硬,体积缩小,表面呈黑褐色.结论对较大的子宫肌瘤进行多点、多面的射频治疗,对较小的子宫肌瘤要控制时间和射频次数.

Abstract:To explore the effect and safeness of the cold circulation radio- frequency (RF) in thawing out myoma of uterus in vitro. In treating 30 individual or frequently-occurred cases of myoma of uterus in vitro with RF ablation and solidifying technique, comparative studies were made to the RF practice with regard to the duration control, the times of RF administration and the surface temperature in accordance with the size of tumor. We found that there was a oval or circular shape gray ablation extent in the sectional view of RF range. It became hard. There was a obvious boundary between the edge of the Rf range and muscle. And chorion was separated from the myoma. If the distance between needle point and the surface of myoma was farther than 1.5 cm, there was no change on the surface. If it was nearer than 1.0 cm, the envelope of the myoma would swell. Then the myoma would be dehydrated and be brown. If RF longer, the myoma would be hard and be contracted. The surface of it would be black brown. Conclusion] It is recommended to employ RF for the multi-spot and multi- stretch treatment for the massive myoma of uterus. It is recommended to control the duration and the times of RF administration for smaller myoma of uterus.
