妇科腹腔镜手术预防性抗生素应用探讨

目的 探讨妇科腹腔镜手术预防性抗生素的合理应用。方法 以2005年2～9月在该中心住院治疗的80例腹腔镜手术患者为研究对象，随机分为2组。观察组中卵巢囊肿、子宫纵隔、宫腔粘连、不孕症各10例，只于手术前30min静脉点滴单剂抗生素，对照组病种和例数与观察组相同，于手术前30min给予单荆抗生素后继续静脉点滴抗生素3d。就2组术后肠功能恢复时间、术后体温、白细胞、中性粒细胞计数、住院时问、住院费用等进行比较。结果 比较2组术后肠功能恢复时间、术后体温、白细胞计数及住院时间等，差异无显著性，抗生素和住院费用比较，2组差异有显著性。观察组较对照组抗生素和住院费用便宜。结论 对妇科腹腔镜清洁手术术前0．5h给单剂抗生素即可达到预防感染的目的，术后无需继续使用抗生素。

Search after using prophylactic antibiotics of gynaecological laparoscopy

[Objective] To search after reasonable using prophylactic antibiotics of gynaecological laparoscopy. [Methods] From Feb. to Sept. 2005, 80 women who had performed laparoscopy in Hysteroscopic Center were divided into 2 groups. Observational Group included ovarian cyst 10 cases, uterine septa 10 cases, uterine adhesion 10 cases and infertility 10 cases. They just used single dose of antibiotics intravenously 30 min before surgery. Control Group included 40 cases which diagnosis was the same with observational group. They used single dose of antibiotics intravenously 30 min before surgery and prolonged to 3 days after surgery. The recovery time of bowel function, degree of postoperative temperature. white blood cell count hospital stay, expenses of antibiotics & hospitalization were compared with two groups. [Results] There is significant difference of recovery time of bowel function, degree of postoperative temperature. white blood cell count and hospital stay between two groups. As for expenses of antibiotics & hospitalization between two groups the difference is obviously. For observational Group the expenses of antibiotics & hospitalization were cheaper than control group. [Conclusions] For clean laparoscopic surgery single dose antibiotics injected intravenously 30 min before surgery can prevent postoperative infection. After surgery antibiotics is not needed
