乙型肝炎肝硬化患者血清IL-17 IL-23 CRP三者关系的探讨

目的：探讨乙型肝炎肝硬化患者血清白介素17（ interleukin17，IL-17）、白介素23（ interleukin23，IL-23）、C反应蛋白（ C-reactive protein，CRP）水平的变化及其意义。方法收集10例健康体检者作为正常对照，40例慢性乙型肝炎，38例代偿期肝硬化，40例失代偿期肝硬化。应用酶联免疫分析法（ ELISA）及免疫比浊法测定各组患者血清中IL-17、IL-23和CRP水平并进行比较。结果①各组患者与正常对照组血清IL-17、IL-23和CRP水平差异均有统计学意义（P＜0．05）。代偿期肝硬化组、失代偿期肝硬化组和慢性乙型肝炎组之间比较差异有统计学意义（P＜0．05）。②乙型肝炎肝硬化患者血清IL-17和IL-23、CRP呈正相关。结论①乙型肝炎肝硬化患者血清IL-17水平远高于正常人，与公认的反应炎症状态的指标CRP呈正相关性，提示IL-17可能参与了乙型肝炎肝硬化的慢性炎症和肝纤维化；②IL-17和IL-23表达水平有密切相关性，IL-23可能通过诱导 IL-17的产生而使后者在乙型肝炎肝硬化的发病中发挥作用。

Exploration of IL-17 amount and its correlation to IL-23 and CRP in patients with HBV liver cirrhosis

Objective To observe the changes of and correlation between IL-17 and IL-23 for exploring the relation between IL-17, IL-23,immune anomaly and chronic inflammation in patients with HBV liver cirrhosis.Methods IL-17 and IL-23 in serum of 40 cases of de-compensated liver cirrhosis,38 cases of compensatory liver cirrhosis,40 cases of chronic hepatitis B and 10 cases of healthy control were measured by sandwich ELISA.The levels of CRP were measured by immune turbidimetry.Results ① Serum IL-l7,IL-23 and CRP in pa-tients were much higher than those in normal group (P<0.05),and the expressions of IL-17,IL-23 and CRP in patients showed statistical significance among different groups (P<0.05 ).② IL-17 was positively correlated to IL-23 (r =0.613, P <0.05) and CRP (r =0.551,P <0.05).Conclusion ①IL-17 in patients is much higher than that in normal people,and is positively correlated to CRP,the ac-knowledged marker of inflammation status,suggesting that IL-17 plays an important role in pathogenesis of chronic inflammation and liver fi-brosis in patients with HBV liver cirrhosis and chronic HBV infection.②IL-17 is positively correlated to IL-23,suggesting that IL-23 may play a critical role in the development of HBV liver cirrhosis through inducing the production of IL-17.
