大剂量通液法治疗输卵管性不孕症的临床研究

目的：通过临床试验评价经宫腔镜输卵管插管大剂量通液法治疗输卵管性不孕症的有效率。方法：对经子宫输卵管造影(HSG)诊断为输卵管堵塞的32例袭者(59条输卵管)，先行宫腔镜下输卵管插管常规剂量通液法治疗，无效者再行大剂量通液法治疗。结果：32例中经输卵管插管紫规剂量通液14例(26条)通畅，18例(33条)不通，占55．9％。经大剂量通液法治疗后，14例(28条)通畅，4例(5条)不通，占15．2％，与常规剂量通液法相比有效率明显提高(P＜0．01)。经随访，术后妊娠8例。结论：经宫腔镜输卵管插管大剂量通液法治疗输卵管性不孕症能提高术后输卵管通畅率，无不良反应，是治疗输卵管性不孕的有效方法。

A Clinical Study on Large Volume Hysteroscopic Hydrotubation in Treating Tubal Infertility

Objective: Our aim was to evaluate the effectiveness of large volume hysteroscopic hydrotubation in treating tubal infertility. Methods: Thirty-two patients (59 oviducts) with tubal obstruction diagnosed by hysterosalpingog-raphy ( HSG) were firstly treated with normal volume hysteroscopic hydrotubation. Then we used large volume hysteroscopic hydrolubation to treat those which oviducts have not been cleared by normal volume hydrotubation. Results: Twenty-six oviducts (14 patients) of the 32 patients were cleared by normal volume hysteroscopic hydrotubation, 33 oviducts (18 patients) of the 32 patients (55. 9% ) were still obstructed. After treated by large volume hysteroscopic hydrotubation, 28 oviducts ( 14 patients) of the 18 patients were cleared, 5 oviducts (4 patients) of the 18 patients (15. 2% ) were obstructed. The effectiveness of large volume hysteroscopic hydrotubation was obviously higher than that of the normal ( P < 0. 01 ). Eight of them pregnanted post-operatively. Conclusion: Large volume hysteroscopic hydrotubation treating tubal infertility can elevate the post-operative cleared rate of oviducts without adverse effects. It is an effective method for treating tubal infertility.
