首次剖宫产的不同术式对二次剖宫产的影响

目的:探讨首次剖宫产的两不同的手术方式对第二次剖宫产产生的影响。方法:90例患者分成传统手术组(A组)、新式手术组(B组),各45例,A组第1次手术术式为传统子宫下段剖宫产术,B组第1次手术术式为新式剖宫产术,对两组二次手术盆腔粘连情况、切皮至胎头娩出时间(T1)、手术总时间(T2)、出血量、伤口愈合效果等数据进行比较。结果:A组患者盆腔粘连明显较B组粘连轻(P<0.01)。A组在切皮至胎头娩出时间(T1)、手术总时间(T2)、出血量明显少于B组(P<0.01)。A组伤口愈合效果与B组比较,A组伤口愈合效果差(P<0.05)。结论:首次选择新式剖宫产者,二次手术时盆腔粘连严重,术中出血多,在临床中,应结合患者情况,严格掌握剖宫产指征,选择合适的首次剖宫产手术方式。

The Effect of the First Various Cesarean Section Techniques to Second Cesarean Section

Objective:To discuss the impact of the first various section types to the second cesarean section.Methods:The 90 cases of second cesarean section were reviewed and analyzed.The traditional surgery for lower uterine cesarean section is 45 cases(group A),the new type cesarean section is 45 cases(group B),comparing two abdominal surgery adhesion,The skin incision to the fetal head delivery time(T1),operative time(T2),bleeding,wound healing effect.Results:The traditional surgery for lower uterine cesarean section,operation time was shorter,sticky rates,even with the new type cesarean section the comparative differences were statistically significant(P<0.01),the traditional surgery for lower uterine cesarean section compaered with the new type cesarean section,The skin incision to the fetal head delivery time(T1)and operative time(T2)is shorter,the average blood is less.The comparative differences were statistically significant(P<0.01).Wound healing effect in group A is lower compared with groupB,the comparative differences were statistically significant(P<0.05).Conclusion:Repeated cesarean section surgery of abdominal adhesions and bleeding is more,poor recovery after surgery,the indication
