复方利多卡因乳膏表面麻醉在无痛分娩中的应用

目的观察复方利多卡因乳膏应用于无痛分娩的临床效果及安全性。方法将100例住院分娩的足月初产妇分为观察组及对照组各50例。对照组采用0.15%罗哌卡因+1～2μg/ml芬太尼行硬膜外分娩镇痛;观察组在实施硬膜外镇痛的同时进行宫颈复方利多卡因乳膏表面麻醉。观察两组的疼痛程度、产程时间、难产率、产后出血量及新生儿窒息率。结果观察组的疼痛程度明显轻于对照组(P<0.05),其产程时间、难产率、产后出血量及新生儿窒息率亦明显低于对照组(P<0.05)。结论复方了利多卡因乳膏表面麻醉能明显增强无痛分娩镇痛效果,提高无痛分娩的安全性。

Application of the topical anesthesia with compound lidocaine cream in labor analgesia

Objective To observe the clinical effect and safety of compound lidocaine cream in labor analgesia. Methods One hundred primipara were randomly divided into simple labor analgesia group （ Group S, n = 50 ） and compound labor analgesia group （Group C ,n=50）. Parturients in group S received epidural labor analgesia with 0.15% ropivacaine and 1 -2 μg/ml fentanyl. Mean- while ,Parturients in group C received topical anesthesia with compound lidocaine cream, including the same epidural analgesia. The pain degree, duration of labor, dystoeia rate, amount of postpartum hemorrhage and neonatal asphyxia rate were evaluated. Results The pain degree in Group C is more lower than that in Group S （ P 〈 0.05 ）. The duration of labor is shorter in Group C （ P 〈 0.05 ）. The dystocia rate, amount of postpartum hemorrhage and neonatal aspHyxia rate is less in the former （ P 〈 0.05 ）. Conclusion The topical anesthesia with compound lidocaine cream has a beneficial effect on labor analgesia.
