儿童白内障摘出联合人工晶状体植入术临床分析

目的评价儿童白内障摘出联合人工晶状体植入术的效果、并发症及防治措施。方法选择12岁以下的白内障术后患儿共43例(55眼),其中先天性白内障18例(30眼),外伤性白内障25例(25眼)。定期检查最佳矫正视力,双眼视功能,远期屈光状态和手术并发症,术后随访3~12月。结果视力均有不同程度提高。外伤性白内障组≥0.6者17眼(68.00%),先天性白内障组≥0.6者13眼(43.33%),术后常见的并发症为葡萄膜炎,后囊浑浊,角膜水肿等。后囊浑浊是造成视力下降的主要因素。结论儿童白内障及时诊断、早期手术,采用白内障摘出联合人工晶状体植入可以获得较好的视功能。后发障是主要的术后并发症。长期随访很重要。术后后发障等并发症和弱视治疗是获得良好视功能的关键。

Clinical analysis of pediatric cataract surgery

Objective To evaluate Clinical effect and complications after cataract extraction combined with intraocular lens implantation for pediatric cataract and suggest preventive methods,Methods Forty-three pediatric cataract cases(55eyes) less than 12years old underwent cataract extraction combined with intraocular lens implantation. Patients include 18 congenital cataract cases(3Oeyes) and 25 traumatic cataract cases(25eyes).Best corrected visual acuity,binocular visual function,long-dated refactive status and postoperative complication of everypatient were checked after operation regularly.Follow-up time is from 3m to 12m.Results Seventeen cases(68%) eyesights in traumaticcataract children and 13 cases(46.9%) are morethan 0.5.There are corneal edema,iritis and soon.posterior capsular opacification is themajor cause of eyesight reduction.ConclusionsEarly diagnosis and operation are important,cataract extraction with IOL implantation iseffective for pediatric cataract.PCO is themajor postoperative complication.We emphasizethe importance of long-term follow-up.The treatment of complication and amblyopia are essential to good vision.
