血清cTnI对小儿病毒性心肌炎心肌损伤的诊断价值

目的：评价血清心肌肌钙蛋白I（cTnI）水平对小儿病毒性心肌炎的临床诊断价值。方法：应用免疫化学发光测定法检测心肌炎患儿急性组（n=46）、迁延组（n=20）血清cTnI、CKMB浓度。结果：①急性组cTnI浓度明显增高，与其他两组比较差异有显性意义（P＜0.01），迁延组与对照组比较差异无显性意义（P＞0.05）。②急性组cTnI阳性率为73.91%（34／46），明显高于CKMB的43.48%（20／46），x^2检验两差异有显性意义（P＜0.05）。③2例重症心肌炎、血清cTnI浓度明显增高，分别为13.22μg/L和9.83μg/L，1例死亡，另1例存活。结论：cTnI对病毒性心肌炎急性期心肌损伤诊断有较高的特异性、敏感性，可能对预后判断有帮助，值得临床推广。

An assessment on serum cardiac troponin I for detecting cardiac injury in viral myocarditis in children

Objective：To explore the clinical evaluation of serum cardiac troponin I(cTnI) in the diagnosis of cardiac injury in viral myocarditis.Methods：Concentrations of serum cTnI and Creatine kinase isoenzyme(CKMB) were measured in the control group (n=20) , the deferment group (n=24) and the acute group (n=46) with chemiluminescent Immunoassay (CLIA).Results：①The concentrations of cTnI and CKMB obviously elevated in the acute group .There were significant statistical differences in the concentrations of serum cTnⅠ and CKMB( P<0.01) between the acute group and the other two groups. There was no statistical difference between the control group and the deferment group (P>0.05);② The rate of positive of cTnI 73.91%(34/46) was obviously higher than CKMB 43.48%(20/46).There was significant difference between them(P<0.05).③The concentrations of cTnI of 2 severe cases were obviously elevating (13.22ug/L、 9.83ug/L), one died and the other survived.Conclusion：The method of cTnI,with its specificity and sensitivity,may be helpful in the diagnosis and prognosis of cardiac injury in viral myocarditis.
