解脲支原体的病原学诊断方法初探

摘    要:目的:研究液体培养解脲支原体后加做固体培养的必要性。方法:将180例不孕不育患者标本先接种解脲支原体液体培养基进行液体培养24 h后,再分别转种解脲支原体固体培养基培养,同时取124例液体培养液变红的标本做普通细菌培养和真菌培养。结果:液体培养阳性率为51.11%,固体培养阳性率为46.11%,差异无统计学意义(P>0.05)。但液体培养阳性而固体培养阴性的标本占19.6%,液体培养法判断为阴性的污染标本中28.1%固体培养阳性。普通细菌培养和真菌培养共检出13株产脲酶菌和40株不产脲酶菌。结论:采用单一的液体培养法进行临床检测解脲支原体易出现假阳性或假阴性。确切的诊断仍要靠液体培养初筛后加做固体培养。

Abstract:Objective:To assess the essentiality of culturing Ureaplasma urealyticum(Uu)in liquid medium followed by solid medium enhancement.Methods: Initially,the specimens from 180 cases of infertility were inoculated in the broth for Uu cultivation.After 24 h,the total broth was transferred to solid medium for Uu culturing.At the same time,liquid erubescent specimens were taken in 124 cases for the common bacterial culture and fungal culture.Results: Positive reaction was 51.11% in the liquid culture medium and 46.11% in the solid culture,which suggested no significant difference(P>0.05).However,19.6% specimens were confirmed negative out of the original positive results in liquid culturing,and 28.1% contaminated specimens,formerly determined negative in broth,were confirmed positively in solid cultivation.By detection of the common bacteria and fungi,13 strains of urease-producing bacteria and 40 strains of non-urease producing bacteria were seen in the total specimens.Conclusion: Clinical detection of Uu by simple broth cultivation may lead to false positive or false negative results,thus,definite diagnosis shall rely on preliminary screening by broth cultivation with additional solid culture validation.
