社区路径干预脑血管病疗效观察

目的观察社区路径干预对脑血管病预后的影响。方法将社区脑血管病患者168例随机分为路径干预组85例和普通干预组83例,进行动态观察,比较两组血脂、血压、血小板聚集率、纤维蛋白原水平的变化及脑卒中的发病率、健康知晓率、危险因素控制率及依从性等。结果干预后,路径干预组较普通干预组血脂、血压、血小板聚集率、纤维蛋白原水平及脑卒中的发病率均有明显的下降,健康知晓率、危险因素控制率及依从性显著提高,差异有统计学意义（P〈0.05）。结论对社区脑血管病患者进行路径干预,使社区干预更加规范化、系统化、流程化,是脑血管病社区干预质量和效果的基础保证。

Clinical observation of community path on cerebrovascular disease intervention

Objective To observe influence of community intervention on cerebrovaseular disease prognosis. Methods One hundred and sixty-eight cases of eerebrovascular disease patients community-dwelling were randomly divided into path intervention group （85 cases）and common control intervention group （83 cases）. Parameters of lipids,blood pressure,platelet aggregation rate, fibrinogen levels as well as incidence of stroke,health awareness,risk factors,control rates amt compliance were observed. Results Parameters of lipids,blood pressure,platelet aggregation rate,fibrinogen levels in the intervention group were significantly reduced （P〈0.05）,also the health awareness,risk factors control rates and compliance increased significantly （P〈0.05）compared with conlrol group respectively. Conclusions For the community-dwelling cerebmvascular disease intervention,the community paths help intervention more standardized, systematic,processing and is the basis of＂ quality and effect guarantee.
