| 摘 要： | 对7例严重充血性心力衰竭患者观察了静脉输入消心痛的急性血液动力学效应。以4mg/小时的速度静脉输入消心痛。结果显示,肺楔压和右心房压明显下降,肺动脉压和外周血管阻力轻度下降,心指数、每搏功指数及每搏容积指数均增加,心率和平均动脉血压无明显变化。本研究表明静脉输入消心痛以降低前负荷为主,亦有减轻后负荷作用,用于严重充血性心力衰竭可使心功能得到改善。

| Abstract: | Acute hemodynamic effects of intravenous isosorbide dinitrate were studied in 7patients with severe congestive heart failure(CHF)poorly controlled by digitalisand diuretics. After administration of the drug in 4mg/hr dose,the PWP and RAPdecreased significantly,and MPAP and SVR decreased slightly,CI,SVI and SWIincreased,and HR, MAP and PVR did not change significantly.It was shown thatintravenous isosorbide dinitrate might lead to the dilatation of the venous capaci-tance bed with a lesser effect on the arterial resistance bed,therefore improvingcardiac performance in patients with severe CHF. |
