冠状动脉狭窄患者NT—proBNP/hs—CRP与心脏功能的相关性研究

摘    要:目的研究冠脉狭窄患者N-末端脑钠肽前体(NT-proBNP)和超敏C反应蛋白(hs-CRP)的变化,并探讨其与心功能的相关性。方法纳入急性心肌梗死(AMI)患者30例,不稳定性心绞痛(UAP)患者30例,稳定性心绞痛(AP)患者30例,同期纳入健康体检者30例为对照组,测定各组血清NT-proBNP浓度,hs-CRP浓度、空腹血糖(FBG)浓度及瘦素(LP)的浓度,左心室射血分数(LVEF)、左心室舒张末期内径(LVDd),并对血清指标和心功能指标进行相关性分析。结果①AMI及UAP组的NT-proBNP、hs-CRP、FBG及LP均显著高于AP组及正常对照组(P〈0.05);②AMI组及UAP组的LVDD显著高于AP组及正常对照组,而LVEF显著低于AP组及正常对照组(P〈0.05);③AMI组和UAP组的NT-proBNP、hs-CRP、FBG水平均与LVDD呈正相关,与LVEF呈负相关;④AP及对照组的LVDD及LVEF与NT-proBNP、hs-CRP、FBG及LP水平无相关性。结论 NT-proBNP和hs-CRP与AMI及UAP患者的心功能和其预后关系密切。

Correlation between NT-proBNP/hs-CRP and heart function of coronary stenosis

Abstract:Objective To study the changes of N-terminal pro-brain natriuretic peptide ( NT-proBNP ) and high sensitivity C-reactive protein ( hs-CRP ) in the patients with coronary stenosis, and discuss the correlation between these changes and heart function. Methods The patients with acute myocardial infarction ( AMI group ) , patients with unstable angina pectoris ( UAP group ) and patients with stable angina pectoris ( AP group, each n =30 ) were selected and at the same time 30 health subjects were selected as control group. The concentration of serum NT-proBNP, hs-CRP, fasting blood glucose ( FBG ) and Leptin were detected, and changes of left ventricular ejection fraction ( LVEF ) and left ventricular end-diastolic inner diameter ( LVDd ) were observed. The correlation between serum indexes and heart function indexes were analyzed. Results ① The concentration of NT-proBNP, hs-CRP, FBG and Leptin were all significantly higher in AMI group and UAP group than those in AP group and control group. ② LVDd was significantly higher and LVEF was significantly lower in AMI group and UAP group than those in AP group and control group. ③ In AMI group and UAP group, the concentration of NT-proBNP, hs-CRP and FBG were positively related to LVDd and negatively related to LVEF. ④ In AP group and control group, LVDd and LVEF were not correlated to the concentration of NT-proBNP, hs- CRP, FBG and Leptin. Conclusion The concentration of NT-proBNP and hs-CRP are closely related to heart function and prognosis in the patients with AMI and UAP.
