超声心动图评价高血压病右室结构功能

摘    要:评价原发性高血压右室结构功能。材料和方法:31例原发性高血压患者用超声心动图测量右室内径和右室功能,对照组为31名健康人,观测舌下含化硝苯吡啶对右室舒张功能的急性作用。结果:和正常组相比,高血压病组E/A比值、E峰速度、每搏校正的最大峰充盈率(NPFR)下降,A峰速度、心房充盈分数(AFF)升高,右室、左室舒张功能各参数密切相关,部份高血压病患者存在右室肥厚,舌下含化硝苯吡啶可使高血压病患右室E峰速度、E/A比值升高,AFF下降,差异均有显著性。结论:当肺动脉压力和右室收缩功能均正常时,高血压病患者已存在右室舒张功能障碍,舌下含化硝苯吡啶可改善右室舒张功能。

Abstract:To assess right ventricular structure and function in essential hroertension. Materials andMethods: We examined 31 hypertensive patients and 31 normal subjects using echocardiography and observed acute effect of subligual nifidipine on right ventricular filling. Besults: The ratio of E/A, peakearly filling velocity, normalized peak filling nde (NPFR) of the right ventricle in HP were lower, butpeak late filling velocity, atrial filling fraction (AFF) were higher than those of noha subjects. Rightventricular filling parametrs were significantly related to these of left ventricle. Right ventricular hypertrophyoccured in part of patients with HP. Sublingual nifidipine intervention significantly increasedthe ratio of E/A, peak early filling velocity, but decreased AFF. Conclusion: Right ventricular diastolicfunction in HP was damaged when right ventricular systolic function and pulmonary pressure were normal. Sublingual nifidipine intervention rovided a beneficial effect on right ventricular diastoic function.
