摘    要:介绍一种经植于皮下的药物输入系统(DDD)作直接门静脉造影CT(CTP)的方法,评价其在肝癌术后复查中的作用。材料和方法:对11例因肝癌手术中植入DDD的患者作CTP复查,用7号针头直接穿刺DDD后,以1ml/S的速度注入浓度为300mgI/ml的造影剂,20-25秒钟后用螺旋CT作全肝足头向扫描。结果:7例DDS的导管头置于门静脉的主干或脾静脉者,肝组织显著强化,而肿瘤病灶内部无明显强化,发现的最小病灶直径为3mm,与常规CT增强扫描相比,显示病灶更多、更为清晰,4例导管头置于门静脉分支者显示病灶的效果较差,本组无不良反应发生。结论:对肝癌术中植有DDD且导管头置于门静脉主干者,可常规作CTP复查。

Abstract:To introduce a direct CT portographic (CTP) method through drug delivery device(DDD) andevaluate the role of this method in the follow uo of the patients with hepatoma. Materials and Methods:CTP was performed in eleven patients with hepatoma operated and implanted with DDD. A 22G needlewas punctured directly into the DDD, and contrast medium of 300mg I/ml was injected at a speed of 1ml/sec. The whole liver was scanned with caudocraniai table incrementation, swting at 20-25sec after thebeginning of contrast medium injection with spiral CT. Results: In 7 Patiens with catheter of DDD inserted into the main portal vena or spleen vena, the liver was enhaned excellently and hepatomas wereenhanced insignificantly. The minimun diameter of lesions in this group was 3mm. Comparing with conventional enhanement CT, CTP demonstraded lesions more and in number much clearer. In the other 4patients with Catheter implated into the branches of portal vena,the lesions of hepatoma were demonstraded insatisfartorily. No adverse reation happened in this group. Conclusion: CTP should be used asa conventional follow up examination for the patients with hepatoma operated and DDD implated intomain portal vena.
