连续血液滤过治疗妊娠合并重症急性胰腺炎的临床疗效观察

目的探讨连续性血液滤过治疗在妊娠合并重症急性胰腺炎的临床应用价值。方法通过对10例妊娠合并重症急性胰腺炎的患者行连续性血液滤过治疗，观察治疗前后体温、平均动脉压、氧合指数、心率、血尿素氮、血肌酐、电解质、C反应蛋白、APACHEⅡ评分的变化。结果8例痊愈，1例死亡，1例自动放弃治疗出院。治疗后血尿素氮、血肌酐、血淀粉酶、C反应蛋白、甘油三酯和APACHEⅡ评分较治疗前明显下降（P〈0．05）；血电解质、血白细胞数及血pH值正常。结论妊娠合并重症急性胰腺炎早期行连续性血液滤过可明显改善临床症状，控制炎症反应，预防发生多脏器功能衰竭，有效降低母婴死亡率。

Observation on continous veno-venous hemofiltration for pregnancy patients with severe acute pancreatitis

Objective To evaluate the clinical utility of continous veno-venous hemofiltration （CVVH） in the treatment of severe acute pancreatitis in pregnancy patients. Methods After treating 10 pregnant patients with severe acute pancreatitis using CVVH, the body temperature, mean arterial pressure, oxygenation index, heart rate, blood urea nitrogen, serum creatinine, elec. trolytes, C-reactive protein, APACHE Ⅱ score changes were assessed. Results Eight patients recovered, one patient died, one patient gave up as economic reasons. Blood urea nitrogen, serum creatinine, serum amylase, C-reactive protein, triglyceride, APACHE Ⅱ score were significantly decreased than that before treatment （P 〈0.05）; blood electrolytes, blood leukocyte count and blood pH were normal. Conclusion The early use CVVH for pregnancy patients with severe acute pancreatitis could significantly improve the clinical symptoms, control inflammation, prevent the occurrence of multiple organ failure, reduce maternal and child mortality.
