BiPAP通气治疗重症哮喘临床效果分析

目的 探讨面罩下双水平气道正压（BiPAP）通气在重症哮喘患者治疗中的意义。方法回顾性分析我院2003年4月至2005年6月住院治疗的48例重症哮喘患者的临床资料。治疗组25例，在常规治疗（吸氧、解痉、平喘、水化、应用激素、纠正酸碱失衡及抗感染等）基础上使用BiPAP通气治疗；对照组23例，使用常规治疗。监测治疗组通气治疗前后临床表现及呼吸（RR）、心率（HR）、平均动脉压（MAP）、动脉血气分析等指标的的变化，并与对照组作比较。结果 治疗组与对照组治疗后各参数（除pH外）比较差异有显著性意义（P〈0．05），临床症状明显改善。结论 在常规治疗的基础上早期应用BiPAP呼吸机治疗重症哮喘，可取得较好疗效。

Clinical efficacy of BiPAP ventilation for severe asthma

Objective To discuss the values of face-masked double horizontal positive airway pressure(BiPAP) ventilation in the treatment of severe asthma.Methods Clinical data of 48 cases of severe asthma were retrospectively analyzed who were treated in our hospital from April 2003 to June 2005.In the treatment group,25 patients were given BiPAP treatment besides routine remedies including oxygen inhalation,spasmolytic and antiasthmatic medication,hydration,hormone,correction of acid-base balance and anti-infection.In the control group,23 patients were only given the same routine remedies as those in the treatment group.The respiration rate,heart rate,average artery pressure,and arterial blood-gas were monitored and compared between the two groups.Results All the parameters except pH showed significant difference between the two groups(P < 0.05).Clinical signs were remarkably ameliorated in the treatment group.Conclusion Early application of BiPAP on the basis of conventional remedies is favorable to reach a good curative effect for such patients.
