摘    要:研究脉动脉压力对肺小动脉璧原弹性蛋白表达与颁的影响以帮助阐明先天性心脏病(先心病)肺动脉高压(肺高压)肺血管重建的发病机制。将15例患儿分为对照组(组1)、先心病高肺血流组(组2)及先心病肺高压组(组3)。用免疫组化技术研究弹性蛋白在肺小动脉璧的含量与分布以了解弹性蛋白的合成情况;用原位杂交技术在转录水平研究肺小动脉间性蛋白mRNA的表达分布。结果:各组患儿间年龄比较无差异。组2、组3间肺动脉压力

Changes of elastin expression in the pulmonary arteries during pulmonary hypertension

Abstract:To explore the pathogenesis of pulmonary vascular remodeling in the existence of pulmonary hypertension (PAP) complicated by congenital heart diseases (CHD) in children. The expression of tropoelastin mRNA by in situ hybridization and the content and distribution of elastin by immunohistochemical assay in the wall of pulmonary arteriole were detected in 15 patients who were randomly divided into control group (group 1), high lung blood flow group(group 2)and hypertension group(group 3). The results showed that no significant difference of age were found among three groups. There was a significant difference in PAP(P<0. 05) and no difference in Qp/Qs (P>0. 05) between group 2 and 3. The tropoelastin mRNA expression of different degrees, that were correlated with PAP (r = 0. 89,P<0. 01) but not with Qp/ Qs (r=-0. 59,P = 0. 08) were found in group3. It was further discovered that positive expression was mainly located at the adventitia and outer regions of pulmonary arteriole. The positive signals of elastin mainly located at the media of pulmonary arteriole in group 1 and 2, and at the adventitia and outer regions of pulmonary arteriole in group 3, respectively. It is concluded that hypertension of pulmonary arteriole, that is main factor to promote the increase of elastin synthesis, can lead to both of re-expression of tropoelastin mRNA and accumulation of elastin in abnormal sites. It will be important to clarity the pathogenesis of pulmonary vascular remodeling in the existence of PAP complicated by CHD in children.
