腰椎间盘突出症合并神经根管狭窄的手术治疗

目的：探讨腰椎间盘突出症合并神经根管狭窄手术显露范围及手术要点，术后腰椎稳定性保护。方法：对１９８５～１９９７年间，因腰椎间盘突出症合并神经根管狭窄在我院进行手术治疗的５９例中的５１例进行６个月～８年的随访，并将其疗效进行分析。结果：优３０例，良１６例，一般５例，优良率为９０２％。结论：针对术中实际情况，采用扩大开窗可以获得满意效果，其优点是基本保留了腰椎后柱结构，在生物力学上保持了脊柱的稳定性。认为术中常规探查神经根管并结合实际进行彻底减压乃是手术的关键步骤。

An Analysis of Surgery-Operative and Curative Effect for Lumbar Disc Herniation with Stenosed Nerve Root Canal

Objective:To inquire into the range of exposure and the main points in the operation and the protects of postoperative stability for lumbar disc herniation with stenosed nerve root canal.Methods:The results of 0.5～8 years' follow up in 51 patients who had undergone operation in our hospital from 1985 to 1997 for lumbar disc herniation with stenosed nerve root canal were analyzed.Results:It was shown that the operative effectiveness was excellent in 30 cases ,good in 16 and fair in 5,the effective rate was 90.2%.Conclusion:The author believes that the way of using enlarging fenestration can produce satisfactory results.The advantage was basic to structure of behind column and stability of spinal column on biomechanics.In operation,the author considers it important to understand nerve root canal and the decompression thoroughly.That is the key to successful operation.
