| 摘 要： | 目的：探索符合临床药师查房要求、简便、合理、连续、有效的药历书写模式。方法：在临床查房期间借鉴国内外药历模式及临床病历,建立住院药历。结果：初步建立了较为实用、清晰、规范、符合查房需要的药历。结论：住院药历的书写模式反映患者用药的整个过程,建立规范化药历可促进临床合理用药,临床药师进一步研究规范化药历的书写及推广具有重要的意义。
