实时三平面超声心动图测量心室容积及心肌重量模型的实验研究

摘    要:目的探讨实时三平面超声心动图(RT-3PE)测量心室容积与心肌重量的可行性与准确性.方法应用RT-3PE采集15个双层橡胶水囊模型的超声图像,沿囊壁勾画并计算内、外囊的容积和囊间液体重量.同时在二维(2DE)超声心动图上采用Simpson's双平面法测定并计算内、外囊的容积和囊间液体重量.上述测值分别与实际注水量对照.结果RT-3PE可实时、直观、真实地显示心室模型的形态.2DE及RT-3PE上内囊容积及囊间液体重量测值与实际值比较均无显著差异(P>0.05),且相关性良好(r为0.919~0.984).R-3PE测量的百分误差与系统偏倚均较2DE小.结论RT-3PE可准确定量测量心室容积和心肌重量,为临床简便、快速地评价心室功能提供了一种新的方法.

Abstract:Objective To evaluate the feasibility and accuracy of ventricular volume and myocardial mass in phantoms measurement by real-time tri-plane echocardiography (RT-3PE). Methods RT-3PE was performed in 15 ventricular wall and myocardium phantoms, which were made of two rubber-bursas. The volumes of the interior bursas and the outer bursas were measured by RT-3PE, and then the masses were calculated. Meanwhile, the volumes were measured with Simpson's method by 2DE. The true volumes were measured by drained water. The volumes by RT-3PE and 2DE were separately compared with the true volumes. Results Spatial images of model were vividly, accurately observed by RT-3PE. There was no significant difference between volumes, mass by 2DE, RT-3PE and the true values (P>0.05). Simple regression showed strong correlation between the measured values and actual values (r= 0.919-0.984). Percent error and the systematic bias by RT-3PE were smaller than those by 2DE. Conclusion RT-3PE could accurately quantify ventricular volume and myocardium mass phantoms and provide a new method to evaluate ventricular function.
