200例正常高值血压中医症状积分的主成分与因子分析

目的：观察滋阴平肝潜阳汤调治阴虚阳亢型正常高值血压的临床疗效。方法：前瞻性、随机对照临床研究。共收集阴虚阳亢型正常高值血压者200例,所有病例随机分为治疗组（100人）与对照组（100人）,分别用滋阴平肝潜阳汤和杞菊地黄丸治疗,并观察两组患者治疗前后血压水平、证候疗效、血脂、血糖等。结果：X1（眩晕）、X4（五心烦热）、X8（舌质）主要由F1（第一公因子）＂阴虚阳亢主证与舌质体征（外象）＂解释,X5（眼花）、X10（脉象）主要由F2＂目窍失养与脉象体征（外象）＂解释,X2（头痛）、X3（腰酸）主要由F3＂清窍与肾府失养＂解释,X6（口干）主要由F4＂津不上承＂解释,X9（舌苔）主要由F5＂舌苔体征（外象）＂解释,X7（耳鸣）主要由F6＂耳窍失濡＂解释。结论：滋阴平肝潜阳汤是治疗阴虚阳亢型正常高值血压的有效方药,不但能降低血压还能改善症状。X1（眩晕）、X4（五心烦热）、X8（舌质）是确定阴虚阳亢证的主要临床依据。

Principal Constituent and Factor Analysis of Symptom Integration of TCM in 200 Patients with High-normal Blood Pressure

Objective： To observe clinical effects of Ziyin Pinggan Qinayang decoction for high-normal blood pressure belongs to type of deficiency of YIN leading to hyper activity of YANG.Methods： Prospective,random contrast clinical study.We observed 200 patients with high-normal blood pressure belong to type of deficiency of YIN leading to hyper activity of YANG,these patients were divided randomly into treatment group（100 cases） and control group（100 cases）,and respectively treated by Ziyin Pinggan Qinayang decoction and Qiju Dihuang Pill,and observe blood pressure levels,therapeutic effect of syndrome,blood lipids,blood glucose,etc of patients in the two group before and after treatment.Results： Principal constituent and factor analysis of symptom integration of TCM： X1（vertigo）,X4（five feverish centres）,X8（texture of tongue）were mainly explained by F1（first common factor）,that is principal syndrome of yin deficiency with yang hyperactivity and physical sign of tongue body（external imagery）,X5（dim eyesight）,X10（pulse condition）were mainly explained by F2,that is malnutrition of eyes and physical sign of pulse condition（external imagery）,X2（headache）,X3（waist soreness）were mainly explained by F3,that is malnutrition of oriffices on head and kidney,X6（Dry mouth）was mainly explained by F4,that is body fluid cannot flow upwards,X9（coated tongue）were mainly explained by F5,that is physical sign of tongue body（external imagery）,X7（tinnitus）were mainly explained by F6,that is malnutrition of ears.Conclusion： Ziyin Pinggan Qinayang decoction is effective for high-normal blood pressure belong to type of deficiency of YIN leading to hyper activity of YANG,it not only lower blood pressure,but also improve symptom.X1（vertigo）,X4（five feverish centres）,X8（tongue body）were main clinical diagnosis criterion for symptom of asthenic yin causing predominant yang.
