家族聚集性慢性乙型肝炎临床特征的研究

目的：探讨家族聚集性慢性乙型肝炎患者（CHB）中医分型与病毒载量及肝组织病理的关系。方法：随机采集临床确诊家族聚集性CHB患者65例和非家族型CHB患者95例,分别进行HBV-DNA载量、肝脏病理检测,并进行中医临床分型。结果：统计结果显示：CHB以湿热中阻为主,家族聚集型与混合型并无显著差异。而且湿热型HBV-DNA载量高（80.0%）,肝脏炎症反应较明显而纤维化程度相对较低。结论：CHB以湿热中阻为主,家族聚集性CHB中医分型与混合型并无显著差异,但家族聚集性CHB病毒高载量、肝脏病理损伤较严重,这些都成为家族性乙型肝炎预后不良的主要因素。

Researeach on the Clinical Features of Familial Aggregation of Chronic Hepatitis

Objective：To observe the relationship between the viral load and the hepatic tissue phathology of familial aggregation of chronic hepatitis B with syndrome differentiation of TCM.Methods：This research abides by the principle of random contrast. 65cases of familial aggregation of CHB and 95 cases of non-familial type CHB are respectively tested HBV-DNA and hepatic tissue pathology,at the same time,all the cases are differentiated by syndromes of TCM. Results：The majority type of CHB is heat and dampness congested type.There has no signficant difference between the two groups.And the heat and dampness congested type has a more viral load（80%）,this type is obvious in liver inflammation ,but is lower in the level of fibrosis. Conclusion：The majority type of CHB is heat and dampness congested type.There has no signficant difference between the two groups.But the familial aggregation of CHB has a more viral load ,and its pathological damage is more severe. These are major factors of familial aggregation of CHB in poor prognosis.
