泉州地区呼吸道病原体谱流行病学研究

摘    要:目的:分析泉州地区呼吸道感染患者非典型病原体的流行趋势及现状,为临床诊断治疗提供参考依据.方法:2017年1月~2019年12月,对泉州市第一医院17544例呼吸道感染患者血清采用间接免疫荧光法(IFA)检测8种非典型呼吸道病原体中的IgM抗体,包括腺病毒(ADV),肺炎支原体(MP),肺炎衣原体(CP),呼吸道合胞病毒(RSV),甲型流感病毒(INFA),乙型流感病毒(INFB),副流感病毒(PIV)及嗜肺军团菌(LP),并对检测结果进行统计分析.结果:在17544例患者标本中检测出IgM抗体阳性6113例,总阳性率为34.84%.女性患者非典型呼吸道病原体抗体阳性率均高于男性患者,其差异有统计学意义(P<0.001).春,冬季节呼吸道病原体IgM抗体阳性率高于夏秋季(P<0.001).不同年龄组间的病原体阳性检出率不同,0~18岁组呼吸道病原体感染阳性率最高为27.11%(P<0.001).6113例阳性患者标本中,检出单一感染5111例,占阳性例数的83.61%.混合感染1002例,以INFA+INFB为主,占阳性例数的16.39%.结论:泉州地区非典型呼吸道病原体感染阳性率有性别,季节,年龄差异性,以单一感染为主,主要在秋冬季节感染.采用间接免疫荧光法可以快速检测出8种非典型呼吸道病原体的IgM抗体,使患者尽早的治疗;另外,根据泉州地区呼吸道的感染情况和流行病学特征,对病原体进行预防和控制.

Epidemiological study of 8 atypical pathogens causing respiratory tract infections in Quanzhou

Abstract:Objective:The epidemic trend and current situation of pathogen in patients with respiratory tract infection in quanzhou were analyzed to provide reference for clinical diagnosis and treatment.Methods:IgM antibodies of 8 respiratory atypical pathogens were detected by indirect immunofluorescence assay(IFA)in the serum of 17544 patients with atypical respiratory infection from January 2017 to December 2019 in the first hospital of Quanzhou region,including adenovirus(ADV),mycoplasma pneumoniae(MP),chlamydia pneumonia(CP),respiratory syncytial virus(RSV),influenza virus A(INFA),influenza virus B(INFB),parainfluenza(PIV)and legioneUa pneumophila(LP).The testing results were analyzed.Results:Among 17544 cases,there were total 6113 positive cases of IgM antibodies against 8 kinds of respiratory pathogens and the positive rate was 34.84%.The positive rate of antibodies against 8 respiratory pathogens in female patients was higher than that in male,the difference was statistically significant(P<0.001).The positive rate of IgM antibody against respiratory pathogen in spring and winter was higher than that in summer and autumn(P<0.001).The positive rate of pathogen was different in different age groups.The highest positive rate of respiratory tract pathogen infection was 27.11%(P<0.001)in the 0-18 years old group.The 5111 cases of single infection were detected,accounting for 83.61%of the positive cases.1002 cases were with mixed infection,and INFA+INFB accounting for 16.39%.Conclusion:The positive rate of atypical respiratory pathogen infection at Quanzhou was different in gender,season and age,infected mainly in autumn and winter.IgM antibodies of 8 respiratory pathogens were detected by indirect immunofluorescence method,so that patients with respiratory infection could get accurate and rapid treatment.According to the infection situation and epidemiological characteristics of respiratory tract infection in Quanzhou area,the pathogen was prevented and controlled.
