内固定治疗胫骨平台骨折的临床分析

目的 探讨胫骨平台骨折内固定治疗的临床效果.方法 胫骨平台骨折11例(Schatzker分型:Ⅲ型1例,V型3例,Ⅵ型7例),均采用切开复位内固定治疗,骨缺损者同时予以植骨.结果 11例患者术后随访6个月至4年(平均1.7年).按Hohl评分标准,优3例、良5例、可2例、差1例,优良率达72.73％.术后并发创伤性关节炎2例,关节强直1例.结论 手术内固定是治疗胫骨平台骨折的有效方法,术后并发症的发生与骨折的严重程度密切相关.保护好骨折段的生物学环境、术中解剖复位并牢固内固定,以及术后早期适当功能锻炼能提高手术疗效.

Internal Fixation for the Treatment of Tibial Plateau Fracture

Objective To investigate the clinical efficacy of open reduction and internal fixation for the treatment of tibial plateau fractures. Methods Eleven cases of tibia1 plateau fracture （Schatzker classification： Ⅲ type 1 case, Ⅴ type 3 cases, Ⅵ type 7 cases） were received the open reduction and internal fixation and allograft implantation for defective fractures. Results All the cases was followed up for 6 months to 5 years （mean 1.7 years）. Based on Hohl scale, 3 cases were excellent, 5 cases were good, 2 cases were fair, 1 case was poor and the acceptable rate was 72.73%. Traumatic arthritis was observed in 2 cases and ankylosis was observed in 1 case. Conclusion Internal fixation is an effective method to treat tibial plateau. Complications after operation are closely associated with the fracture complexity. A good protection of the biological environment, anatomical reduction and early postoperative functional exercise can improve surgical effects.
