超声造影在门静脉栓子良恶性鉴别诊断中的价值

目的探讨超声造影在门静脉栓子良恶性鉴别诊断中的价值。方法采用SonoVue超声造影剂及对比脉冲序列（CPS）技术对22例门静脉癌栓，8例门静脉血栓共30例门静脉栓子进行超声造影，观察造影后栓子内血流增强情况。结果22例门静脉癌栓中，17例动脉相整体均匀性增强，1例低回声增强，4例部分增强。8例门静脉血栓，无1例有增强。超声造影诊断门静脉癌栓的敏感性和特异性高达100％。结论超声造影能非常准确地鉴别出门静脉栓子的良恶性，具有非常重要的临床价值。

Importance of Contrast-enhanced Ultrasonography in Differential Diagnosis of Portal Vein Thrombosis

Objective To evaluate the diagnostic value of contrast-enhanced ultrasonography in differential diagnosis of portal vein thrombosis.Methods SonoVue-enhanced ultrasonography with contrast pulsed sequencing(CPS) imaging technique was performed on 30 patients with portal vein thrombosis,included 22 portal vein tumor thrombosis and 8 portal vein benign thrombosis.Enhancement process were assessed.Results All 22 portal vein tumor thrombosis were variously enhanced.A homogeneous enhanced pattern was detected in 17 portal vein tumor thrombosis.A hypoechoic enhanced pattern was detected in 1 portal vein tumor thrombosis.A heterogeneous enhanced pattern was detected in 4 portal vein tumor thrombosis.All 8 portal vein benign thrombosis were not enhanced.The sensitivity and specificity in diagnosis of portal vein thrombosis with contrast-enhanced ultrasonography were 100%.Conclusions Contrast-enhanced ultrasonography has the ability of accurate differential diagnosis of portal vein thrombosis,and is very important in clinical application.
