类高雪氏细胞在慢性粒细胞白血病中的意义

摘    要:目的:探讨慢性粒细胞白血病(CML)骨髓中出现类高雪氏细胞的意义。方法:利用形态学及细胞化学染色检查94例CML患者和63例类白血病反应患者的骨髓涂片;检测49例CML患者骨髓的Ph染色体;分析94例CML患者出现急变期或加速期的情况。结果:94例CML患者的骨髓涂片中有29例找到类高雪氏细胞,而类白血病反应患者的骨髓涂片中未找到类高雪氏细胞;CML患者Ph染色体的阳性率为91.8%(45/49);65例无类高雪氏细胞的CML有21例处于急变期或加速期(急性期17例,加速期4例),而9例有类高雪氏细胞的CML有2例发生急变,两组间有显著差异(P<0.01)。结论:类高雪氏细胞的出现对CML的诊断及预后有重要价值。

Abstract:Objective:To evaluate the significance of pseudo-Gaucher cells in the bone marrow from patients with chronic myelocytic leukemia (CML).Methods:We examined 157 bone marrow slides(94 from patients with CML and 63 from patients with leukemoid reaction) by morphometry and cytochemistry stain. The Philadelphia chromosome(Ph) was detected in the bone marrow in 49 patients with CML.A study was performed on the CML cases in blast phase and accelerated phase.Results:Pseudo-Gaucher cells were found in the bone marrow in 29 of 94 cases with CML;however,it was not found in the bone marrow in 63 cases with leukemoid reaction.The Ph-positive ratio was 91.8%(45/49).Blast phase or accelerated phase appeared in 21 of 65 CML cases without Pseudo-Gaucher cell and 2 of 29 CML cases with Pseudo-Gaucher cell,and there were significant differences between the two groups( P <0.01).Conclusions:Pseudo-Gaucher cells are of great value to the diagnosis and prognosis of CML.
