摘    要:目的 探讨早期目标导向治疗(EGDT)在治疗感染性休克时对感染性休克相关性急性肾损伤(SSAKI)的预防和治疗作用.方法 100例感染性休克患者分为治疗组和对照组,对照组进行常规治疗,治疗组在早期目标导向治疗上达标.观察患者的临床症状改善情况,常规检测患者血清肌酐(Cr)、尿素氮(BUN)及白细胞介素-6(IL-6)、肿瘤坏死因子-α( TN F-α)水平,观察早期目标导向治疗组和对照组之间SSAKI的发病率和对SSAKI的疗效.结果 治疗组中发病后<24 h及24~72 h患者SSAKI发病率分别为22%和33%,对照组分别为44%和50%,2组比较差异有统计学意义(P<0.05).各治疗阶段早期目标导向治疗组,均较对照组肾损伤程度显著减轻(P<0.05).结论 早期目标导向治疗在治疗感染性休克时可以显著减少SSAKI的发生率,促进SSAKI的好转.

Abstract:Objective To study curative effects of early goal-directed therapy in septic shock associated kidney injury(SSAKI).Methods 100 patients with septic shock divided into two groups: control group and therapeutic group.56 patients in control group were treated with the conventional treatment,while 44 patients in therapeutic group were treated with EGDT.The patients’ clinical symptoms,the levels of serum creatinine(Cr),blood urea nitrogen(BUN),interleukin-6(IL-6) and tumor necrosis factor alpha(TNF-α) in serum were analyzed to compare the differences in the incidence of injury and curative effect of kidney injury between two groups.Results The incidences of SSAKI within 24 hours and 24~72 hours after septic shock were 22% and 33% in therapeutic group,but those in control group were 44% and 50%.There was significant difference between two groups(P<0.05).Compared with control group,the degree of kidney injury was lessened significantly in therapeutic group(P<0.05).Conclusion EGDT can reduce the occurrence of SSAKI and accelerate the recovery of SSAKI.
