单克隆抗体反向间凝试验的建立及其在日本血吸虫病诊断上的应用

ＭｃＡｂ－Ｇ３－ＲＩＨＡ诊断血吸虫病、采用绵羊红细胞，经戊二醛固定、鞣酸鞣化，致敏纯化肠相关ＭｃＡｂ，然后冷冻干燥用于检测宿主血清中循环抗原，并以ＩＨＡ对比检测ＣＡｂ。以之检测６３０份样本，其中正常人２２４例，ＲＩＨＡ假阳性率为４．４％，ＩＨＡ为４．９１％；急性血吸虫病１６５例，ＲＩＨＡ阳性率９８．７９、ＩＨＡ９９．３９％；疫区人群２４１份，ＲＩＨＡ阳性率７１．７８％，ＩＨＡ为８４．２３％；其中粪检阳性者ＲＩＨＡ阳性率８６．６７％（６５／７５），粪检阴性者的阳性率是６５．０６％（１０８／１６６），而ＩＨＡ的阳性率分别为９６．０％（７２／７５），７８．９２％（１３１／１６６），ＥＰＧ≥３０的慢性血吸虫病者ＲＩＨＡ及ＩＨＡ阳性率均为１００．０％，当ＥＰＧ＜３０时，ＲＩＨＡ和ＩＨＡ阳性率分别是６９．２３％、９２．３１％。实验结果显示：急性血吸虫病人群循环抗原ＧＭＲＴ为慢性患者的２１．７６倍，不同感染度的慢性患者循环抗原ＧＭＲＴ较为接近。急、慢性血吸虫病血清中ＣＡｂ显著多于循环抗原。本法操作简便，有较好的特异性与敏感性，适用于血吸虫病循环抗原的检测。

Study on Detection of Circulating Antigen in Schistosomiasis by RIHA with Monoclonal Antibody

Schistosoma Japonicum was diagnosed by McAb-RIHA The process was following. Erythrocytes of sheep fixed with glutaraldehyde,tanned with tannic acid,sensitized with purific ating gut-associated McAb and lyophlized,with which the parasite-oriented circulating antigens was dctermined a total of 630 serum samples were determined by RIHA,among them 224 normal individuals showed 4.46?lse positive rate,165 acute caes 98.79%positive rate,241 chronic cases 71.78%positive rate.A qualitative comparison of RIHA test with kato examination technique showed a high agreement,ie,86.67%. The p0sitive rates for circulating antigen in cases with 0100 were 83.61%and 100%respectively.The results showed that circulating antigens avevage geometric mean of acute schistosomiasis were obvious higher than chromic schistosomiasis i.e.,22.6 times than the chronic cases. But,circulating antigens average geometric mean were similar with chronic schistosomiasis of different influence degree. It suggested that McAb-RIHA test using this reagent shonldbe easy to handle and specific and sensitive enough to be employed for determinating circulating antigens of schistosomiasis.
