分级诊疗视域下广东省基层卫生资源配置效率分析

目的 通过对2013—2018年广东省基层医疗机构及2018年各地级市乡镇卫生院的卫生资源配置效率的评估，为广东省基层医疗机构优化资源配置，及各地级市合理推进分级诊疗制度提供参考。 方法 采用Malmquist模型测算2013—2018年广东省基层医疗卫生资源整体配置效率；利用CRS-CCR和VRS-BCC模型评估各地级市乡镇卫生院的卫生资源配置效率。 结果 2013—2018年，广东省基层医疗机构Malmquist指数为0.963，技术效率和规模效率为1，技术进步为0.963。横向对比2018年广东省各地市乡镇卫生院卫生资源配置效率发现，地域间差异大，多数城市的乡镇卫生院投入足够，但医疗服务量不足。 结论 基层医疗机构应当加强技术创新，提升医疗水平；根据各地市的具体情况采取不同的对策增加基层的诊疗服务量，推进分级诊疗。

Analysis on the allocation efficiency of primary medical institutions in Guangdong from the perspective of hierarchical diagnosis and treatment

Objective To evaluate the allocation efficiency of health resources in primary medical institutions(2013—2018) and township hospitals(2018) in Guangdong province,to provide reference for optimizing the allocation of primary medical institutions in Guangdong province, and to promoting the hierarchical diagnosis and treatment system in all cities. Methods Malmquist model was used to calculate the overall allocation efficiency of primary medical and health resources in Guangdong province from 2013 to 2018. CRS-CCR model, VRS-BCC model were used to evaluate the allocation efficiency of health resources in township hospitals in cities. Results From 2013 to 2018, malmquist index of primary medical institutions in Guangdong province was 0.963, the value of technical efficiency and scale efficiency was 1, and the technical progress was 0.963. Horizontal comparison of health resource allocation efficiency of township health centers in Guangdong province in 2018 showed that there were large regional differences in the allocation efficiency of township health centers. Most cities had sufficient basic level investment, but the amount of medical services was insufficient. Conclusion Primary medical institutions should strengthen technological innovation and improve medical level. According to the specific situation of various cities, different countermeasures should be taken to increase the amount of primary medical services and promote hierarchical diagnosis and treatment.
