青岛成年双生子不同肥胖指标与高血压的关联与遗传度研究

摘    要:目的 探讨青岛地区成年双生子整体性肥胖、腹型肥胖与高血压患病风险之间的关联,并分析遗传因素对肥胖、血压的影响。方法 以青岛市自愿参加调查的225对成年双生子人群为研究对象,于2019年8—9月进行问卷调查和体格检测。采用多因素条件logistic回归分析肥胖与高血压患病的关系,并用组内相关系数法计算肥胖、血压的遗传度。敏感性分析以卵型鉴定者为研究对象进行多因素条件logistic回归。结果 多因素条件logistic回归分析显示,男性整体性超重、肥胖(依据BMI划分)和腹型肥胖(依据WC、WHR、WHtR划分)人群高血压患病风险是正常人群的2.45、4.86倍和2.77、1.99、3.04倍(P<0.001)。男女同卵双生子组内相关系数均高于异卵双生子。男性双生子BMI、WC、WHR、SBP、DBP的遗传度分别为0.31、0.39、0.13、0.50、0.57,女性的遗传度为0.30、0.64、0.44、0.40、0.53。敏感性分析显示男性整体性超重、肥胖(依据BMI划分)和腹型肥胖(依据WC、WHR、WHtR划分)人群高血压患病风险是正常人群的2.39、5.50倍和2.56、1.86、1.67倍。结论 男性整体性肥胖、腹型肥胖者高血压患病风险增加;且不同性别WC、WHR及SBP、DBP的遗传度不同。

Abstract:Abstract: Objective To investigate the association between obesity, abdominal obesity and hypertension, and to explore the heritability of obesity and blood pressure in adult twins. Methods A population of 225 pairs of adult twins who voluntarily participated in the survey in Qingdao was regarded as the study population, and questionnaires and physical tests were conducted in August -September 2019. Multivariate conditional logistic regression was used to analyze the relationship between obesity and hypertension prevalence, and the heritability of obesity and blood pressure was calculated using the within -group correlation coefficient method. Sensitivity analysis was performed with multi -factor conditional logistic regression using egg type identifiers as the study subjects. Results Multivariate conductional logistic regression showed overweight (OR=2.45), obese (OR=4.86) and abdominal obesity (WC: OR=2.77; WHR: OR=1.99; WHtR: OR=3.04) were significantly associated with a higher risk for hypertension in men. The intra -group correlation coefficient of monozygotic twins was higher than that of dizygotic twins. The average heritability of BMI, WC, WHR, SBP, and DBP of males was 0.31,0.39, 0.13, 0.50, and 0.57, respectively, while those of females were 0.30, 0.64, 0.44, 0.40 and 0.53. Sensitivity analysis showed that overall overweight, obesity (classified according to BMI), and abdominal obesity (classified according to WC,WHR, and WHtR) were 2.39, 5.50, 2.56, 1.86, and 1.67 times higher than the normal risk of hypertension. Conclusion The risk of hypertension is increased in men with overall obesity and abdominal obesity, and the heritability of WC, WHR, and SBP and DBP are different between genders.
