超声引导下刮宫术联合宫腔放置水囊压迫法对子宫瘢痕妊娠疗效分析

目的 探讨超声引导下刮宫术联合宫腔放置水囊压迫法应用于子宫瘢痕妊娠治疗的临床效果.方法 选取2013年1月至2016年12月攀枝花市妇幼保健院收治的子宫瘢痕妊娠患者86例为研究对象,按照治疗方法分为观察组与对照组,对照组患者给予常规超声引导下刮宫术,观察组在对照组的基础上给予宫腔放置水囊压迫法止血,对比两组患者术中出血量、术后出血量、月经恢复时间、血β-人绒毛膜促性腺激(β-hCG)水平恢复正常时间、住院时间,并观察两组患者不良反应发生情况.结果 观察组术中出血量、术后出血量均显著低于对照组(t值分别为6.671、5.532,均P<0.05).观察组月经恢复时间、血β-hCG水平恢复正常时间、住院时间均显著低于对照组(t值分别为7.328、5.175、5.312,均P<0.05),两组手术时间无显著性差异(t=0.913,P>0.05).观察组与对照组总不良反应发生率分别为9.30%、25.58%,观察组显著低于对照组(χ2=3.957,P<0.05).结论 在常规超声引导下刮宫术的基础上联用宫腔放置水囊压迫止血法治疗子宫瘢痕妊娠可明显降低患者的出血量,并缩短术后恢复时间,值得临床推广.

Effect of ultrasound guided curettage combined with intrauterine water sac compression on treatment of uterine scar pregnancy

Objective To discuss the clinical effect of ultrasound guided curettage combined with intrauterine water sac compression on treatment of uterine scar pregnancy.Methods A total of 86 cases of uterine scar pregnancy admitted in Panzhihua Maternal and Child Health Care Hospital from January 2013 to December 2016 were selected and divided into observation group and control group according to treatment methods.Patients in the control group were given conventional curettage surgery guided by ultrasound,and patients in the observation group were treated with intrauterine water sac compression hemostasis on basis of the treatment received in the control group.Intraoperative blood loss,postoperative bleeding,menstruation recovery time,time for blood beta human chorionic gonadotropin(β-hCG)level recovering to normal,hospitalization time in two groups were compared, and adverse reactions in both groups were observed.Results Intraoperative blood loss and postoperative bleeding in the observation group were significantly lower than those in the control group(t value was 6.671 and 5.532,respectively,both P< 0.05). Menstruation recovery time,time for β-hCG level back to normal,and length of hospital stay in the observation group were significantly lower than those in the control group(t value was 7.328,5.175 and 5.312,respectively,all P<0.05).There was no significant difference in operation time between two groups(t=0.913,P>0.05).Total adverse reactions rate in the observation group was 9.30%,which was significantly lower than that of 25.58% in the control group(χ2= 3.957,P< 0.05).Conclusion Conventional curettage surgery guided by ultrasound combined with intrauterine water sac compression hemostasis treatment can obviously reduce bleeding amount and shorten postoperative recovery time when used in uterine scar pregnancy patients,which is worthy of clinical promotion.
