集束化护理对ICU留置中心静脉导管患者护理质量及感染预防的影响

目的 探讨集束化护理对ICU留置中心静脉导管患者护理质量及感染预防的影响.方法 选取2013年1月至2015年12月我院ICU留置中心静脉导管患者180例为研究对象,随机分为观察组和对照组各90例.观察组实施集束化护理,对照组实施常规护理.比较两组患者的护理质量及感染预防效果.结果 观察组患者穿刺点感染、导管相关性血流感染率均低于对照组,差异有统计学意义(P<0.05);观察组患者导管脱出、堵管、血栓及非计划性拔管发生率均低于对照组,差异有统计学意义(P<0.05);观察组患者导管留置天数和住院天数均均短于对照组,差异有统计学意义(P<0.05);观察组患者护理满意度明显高于对照组,差异有统计学意义(P<0.05);观察组患者经护理后的各项生活质量评分均明显高于对照组,差异有统计学意义(P<0.05).结论集束化护理能够降低和预防ICU留置中心静脉导管患者的感染率及其他并发症发生率,减少非计划性拔管发生率,缩短置管时间和住院时间,提高患者护理满意度和生活质量,建议在临床上推广应用.

Analysis of the role of cluster-nursing on the nursing quality and infection prevention of patients inserted with central venous catheter in ICU

Objective To analyze the role of cluster-nursing on the nursing quality and infection prevention of patients inserted with central venous catheter in ICU. Methods A total of 180 cases of ICU patients inserted with cen-tral venous catheter in our hospital from January 2013 to December 2015 were selected as the research objects and were randomly divided into observation group and control group, 90 cases in each group. The observation group was treated with cluster nursing, while the control group received routine care measures. The quality of nursing and prevention of in-fection of patients of the two groups were compared. Results The puncture-site infection, catheter-related bloodstream infection of the observation group were lower than that of the control group, and the differences were statistically signifi-cant (P<0. 05). The incidence of catheter prolapse, plugging, thrombosis and unplanned extubation of the observa-tion group were lower than that of the control group, and the differences were statistically significant (P<0. 05). Time of indwelling of catheters and the length of hospital stay of the observation group days were shorter than that of the control group, and the differences were statistically significant (P<0. 05). The satisfaction for nursing of the observation group was significantly higher than that of the control group, and the difference was statistically significant (P<0. 05). After treatment, the scores of quality of life of the observation group were significantly higher than that of the control group, and the difference between the two groups was statistically significant ( P<0. 05 ) . Conclusions Cluster nursing can reduce and prevent the infection rate of ICU patients inserted with venous catheter and the incidence rate of other compli-cations , meanwhile it can reduce the unplanned extubation of catheter and shorten the indwelling time and length of hos-pital stay, moreover it can improve the satisfaction for nursing and quality of life of patients, which is recommended for the clinical promotion and use.
