儿童原发性肾病综合征肾组织中Toll样受体4及Toll样受体7的表达

摘    要:目的 探究儿童原发性肾病综合征肾组织中Toll样受体4(TLR4)及TLR7的表达及临床意义.方法 选取陕西省西安市儿童医院自2013年7月至2016年7月收治的300例接受肾活检的儿童原发性肾病综合征肾组织作为观察组,按照病例分型可分为MCD型(78例)、MN型(72例)、MsPGN型(75例)及FSGS型(75例),另选择同期收治的70例肾母细胞瘤患儿的正常肾组织作为对照组,采用免疫组化的方法对两组肾组织中TLR4及TLR7的表达水平进行检测.结果 观察组中的不同病理类型组与对照组相比肾小管组织中TLR4的表达水平较高,不同病理类型组织中MCD型肾小管组织中TLR4的表达水平要明显高于其他组,依次是MN型、MsPGN型及FSGS型,差异具有统计学意义(t=4.56~9.23,均P<0.05).观察组中的不同病理类型组与对照组相比肾小管组织中TLR7的表达水平较高,不同病理类型组织中MsPGN型肾小管组织中TLR7的表达水平要明显高于其他组,依次是MN型、MCD型及FSGS型,差异均具有统计学意义(t=4.82~10.25,均P<0.05).结论 TLR4及TLR7在儿童原发性肾病综合征肾组织中呈现较高的表达水平,且其表达水平在不同病理类型中有一定的差异.

Abstract:Objective To probe and analyze the expressions of toll like receptor 4 (TLR4) and TLR7 in renal tissue of children with primary nephrotic syndrome and its clinical significance.Methods Renal tissues of 300 children with primary nephrotic syndrome hospitalized for renal biopsy in Xi'an Children's Hospital from July 2013 to July 2016 were selected as observation group, and according to case classification divided into MCD type (78 cases), MN type (72 cases), MsPGN type (75 cases) and FSGS type (75 cases).Normal renal tissues of 70 children with nephroblastoma hospitalized at same period were chosen as control group.Immunohistochemical method was used to test the expression level of TLR4 and TLR7 in kidney tissues in two groups.Results Expression level of TLR4 in renal tubule tissue in different pathological type of observation group was higher than that in control group, and among different pathological types, expression level of TLR4 in renal tubule tissue in MCD type was significantly higher than that in other types, with MN type, MsPGN and FSGS type followed in sequence, and difference had statistical significance (t value ranged 4.56 to 9.23, all P<0.05).TLR7 expression level in renal tubule tissue in different pathological type of observation group was higher than that in control group, and among different pathological types, TLR7 expression level in renal tubule tissue in MsPGN type was significantly higher than that in other types, with MN type, MCD type and FSGS type followed in sequence, and difference was statistically significant (t value ranged 4.82 to 10.25, all P<0.05).Conclusion Expression levels of TLR4 and TLR7 are high in renal tissue of children with primary nephrotic syndrome, and its expression level has a certain difference in different pathological type.
