经会阴超声对围产期耻骨联合分离的诊断价值

目的 探讨经会阴超声对围产期耻骨联合分离的诊断价值.方法 选择2014年12月至2016年6月在西安医学院第二附属医院的25例孕晚期有明显耻骨联合分离症状的孕妇并进行临床干预,采用经会阴超声检查产前、产后耻骨联合间距,评价干预前后耻骨联合间距与临床症状的相关性.结果 所有孕妇均顺利分娩,其中剖宫产7例,阴道分娩18例,产后耻骨联合分离均有一定程度恢复,经重复测量方差分析,不同耻骨联合间距、VAS评分组内前后比较差异有统计学意义(F值分别为4.447、8.323,均P<0.05);耻骨联合间距>10mm比例和VAS>4分比例组内比较差异同样有统计学意义(χ2值分别为13.844、11.105,均P<0.05);经Spearman相关性分析,耻骨联合间距与VAS评分呈正相关(r=0.613,P=0.034);耻骨联合间距>10mm比例和VAS>4分Kappa检验一致性系数为0.42.结论 耻骨联合间距与疼痛VAS评估评分存在正相关,但间距>10mm与VAS评分>4分无明显相关.

Diagnostic value of transperineal sonography in symphysis pubis diastasis in perinatal women

Objective To investigate the diagnostic value of transperineal sonography in symphysis pubis diastasis in perinatal women.Methods Totally 25 pregnant women in the third trimester with obvious symphysis pubis diastasis were selected and treated by clinical intervention.Prenatal and postnatal pubic symphysis distance was measured by transperineal sonography to evaluate the correlation between pubic symphysis distance and clinical symptoms before and after intervention.Results All pregnant women delivered successfully, including 7 cases of cesarean section and 18 cases of vaginal delivery.Their symphysis pubis diastasis recovered in some degree after birth.After repeated measures analysis of variance, there was statistically significant difference in the different symphysis pubis diastasis and VAS score within the group before and after intervention (F value was 4.447 and 8.323, respectively,both P<0.05).There was significant difference in the ratio of pubic symphysis distance> 10mm and the ratio of VAS > 4 within the group (χ2 value was 13.844 and 11.105, respectively, both P<0.05).Spearman correlation analysis showed that the pubic symphysis distance was positively correlated with the VAS score (r=0.613, P=0.034).The consistency coefficient of the pubic symphysis>10mm and VAS>4 was 0.42.Conclusion There is a positive correlation between the pubic symphysis distance and the VAS assessment score, but the distance> 10mm is not significantly related to the VAS score> 4.
