微生物制剂辅助蓝光治疗新生儿黄疸对患儿生化指标的影响

摘    要:目的 探讨布拉氏酵母菌辅助蓝光治疗新生儿黄疸对患儿转铁蛋白、心肌酶谱及血清总胆红素的影响.方法 选择2016年6到12月在皖北煤电集团总医院新生儿监护室进行新生儿黄疸治疗的患儿60例,按照随机数字的方法,分为对照组和观察组,每组30例,分别给予单纯的蓝光治疗和布拉氏酵母菌辅助蓝光治疗,比较两组的治疗效果.结果 治疗前,对照组和观察组的转铁蛋白、乳酸脱氢酶、肌酸激酶、肌酸激酶同工酶、总胆汁酸、总胆红素和直接胆红素比较差异无统计学意义(均P>0.05).治疗后,对照组和观察组的转铁蛋白均高于治疗前,比较差异有统计学意义(t值分别为5.48、10.49,均P<0.05),两组间比较观察组高于对照组,差异亦有统计学意义(t=4.71,P<0.05);对照组和观察组的乳酸脱氢酶、肌酸激酶、肌酸激酶同工酶、总胆汁酸、总胆红素和直接胆红素均低于治疗前,差异有统计学意义(t值分别为6.71、9.22、3.99、10.16、12.76、9.47、9.17、32.23、22.15、20.79、37.11、28.30,均P<0.05),两组间比较观察组低于对照组,差异亦有统计学意义(t值分别为3.13、4.66、5.35、11.74、28.12、25.49,均P<0.05).两组治疗有效率比较差异具有统计学意义(χ2=9.23,P<0.05),观察组的治疗效果显著优于对照组.结论 利用布拉氏酵母菌辅助蓝光治疗新生儿黄疸,可以使转铁蛋白、心肌酶谱和血清胆红素的指标明显改善,提升治疗有效率,值得在临床上进行推广.

Abstract:Objective To investigate the effect of Saccharomyces boulardii assisting blue light therapy on transferrin , myocardial enzymes and serum total bilirubin in neonatal jaundice .Methods Sixty cases of neonatal jaundice treated in neonatal intensive care unit of Wanbei Coal and Electricity Group General Hospital from June to December in 2016 were selected and randomly divided into control group and observation group with 30 cases in each group . Control group was treated with pure blue light and observation group was given Saccharomyces boulardii assisting blue light treatment .Treatment effects were compared between two groups .Results There was no statistically significant difference in transferrin , lactate dehydrogenase , creatine kinase , creatine kinase isoenzyme , total bile acid, total bilirubin and direct bilirubin between observation group and control group before treatment (all P>0.05).Transferrin in control group and observation group after treatment was higher than that before treatment , and difference was statistically significant ( t value was 5.48 and 10.49, respectively, both P<0.05).Compared between two groups , transferrin in observation group was higher than that in control group, and difference was also statistically significant (t =4.71,P<0.05).Lactate dehydrogenase, creatine kinase, creatine kinase isoenzyme, total bile acid, total bilirubin and direct bilirubin in both groups after treatment were lower than those before treatment , and difference was statistically significant (t value was 6.71, 9.22, 3.99, 10.16, 12.76, 9.47, 9.17, 32.23, 22.15, 20.79, 37.11 and 28.30, respectively, all P<0.05).Comparison between two groups indicated that above parameters in observation group were lower than those in control group , and difference was also statistically significant ( t value was 3.13, 4.66, 5.35, 11.74, 28.12 and 25.49, respectively, all P<0.05).Difference in treatment effective rate in two groups was statistically significant (χ2 =9.23, P <0.05). Treatment effect in observation group was significantly better than that in control group .Conclusion Saccharomyces boulardii assisting blue light therapy for neonatal jaundice can improve transferrin , myocardial enzymes and serum bilirubin indexes and improve treatment efficiency .It is worthy of clinical popularization .
