石家庄地区散发性小儿病毒性肝炎病原学研究

对122例小儿病毒性肝炎患儿血清进行乙肝5项、抗—HAV—IgM、抗—HCV、丁肝3项、抗—HEVIgM、PCRHBV—DNA和HCV—RNA检测,对上述检测阴性患儿血清增检CMV DNA和嗜异凝集抗体。结果:4例(3.3％)患儿上述7种病毒感染标志物皆阴性,暂定为病原不清性病毒性肝炎;95例(77.9％)患儿血清一种肝炎病毒感染标志物阳性,其中甲、乙、丙、丁和戊型肝炎依次为56.8％、33.6％、4.2％、1.1％和4.2％。两种或两种以上肝炎病毒感染者23例(18.9％),其中甲乙和乙丁分别占11/23及3/23;甲丙、乙丙丁和丙戊各占2/23;甲乙丁、乙戊和甲乙戊各占1/23。未检出巨细胞病毒DNA和嗜异凝集抗体。值得注意的是15例慢肝者中,10例为单独乙型肝炎,4例为乙肝与丙肝及丁肝重叠感染,1例为丙型肝炎。

A Etiologk Study of Spot Viral Hepatitis in Infants and Children in SHI JIA ZHUANG Area

To elucidate the viral etiology of viral hepatitis in infants and children, 122 of such patients were tested for IgM-HAV, HBV markers, anti-HCV, HDV markers and IgM-HEV with EIA PCR (HBV -DNA) and RTPCR (HCV-RNA). For those whose tests were negative, CMV-DNA and heterophile antibody were tested in addition. The results showed that 95 (77. 8%) cases were of sole infection, the hepatitis A,B, C. D and E were 54 (44.3%), 32 (26.2%), 4 (3. 3%), 1 (0. 8%)and 4(3. 3%) Cases respectively. 23 (18. 9%) patients were caused by more that double infection: 11 (9. 0%) cases were of HAV and HBV coinfection, 3 (2.4%) cases of HBV and HDV coinfection, 2 (1.6%) cases of HAV-HCV, HBV-HCV -HDV and HCV-HEV coinfection. One (0. 8%) case was of HBV-HEV and HAV-HCV-HEV coinfection. Four (3.3%) cases were not idlentified, the PCR CMV-DNA and heterophile antibody were negative too.
