微柱凝胶试验在冷凝集素检测中的应用

摘    要:目的:探讨微柱凝胶试验在冷凝集素检测中的应用。方法:采用微柱凝胶法、聚凝胺法对2例冷凝集素结果干扰血型鉴定和交叉配血的患者进行ABO血型、Rh血型、交叉配血、直接抗人球蛋白实验、冷自身抗体吸收试验、抗体筛选试验。结果:(1)血型鉴定:两种方法均显示强凝集现象,不能判读血型,但微柱凝胶万能卡孵育再次离心后血型结果显示清晰;(2)交叉配血:2例患者微柱凝胶万能卡上结果显示,主次侧均不凝集,结果显示清晰。而凝聚胺法结果显示,主侧凝集,次侧不凝集;(3)直接抗人球蛋白试验和抗体筛选实验:例1直接抗人球蛋白试验阴性,例2直接抗人球蛋白试验阳性,2例抗体筛选试验均为阴性;(4)冷自身抗体吸收试验:吸收后的血清与自身细胞、A、B、O型红细胞均不出现凝集,正反定型相符;(5)冷凝集素效价:例1效价为1:256;例2效价为1:512。结论:微柱凝胶试验操作简便,重复性好、灵敏度高,便于冷凝集素的分析鉴定。

Abstract:Objective:To investigate the microcolumn gel test in cold agglutinin detection application.Methods:Using micro column gel method,Polybrene method in 2 cases of cold agglutinin results from disturbance of blood type identification and cross matching with ABO blood group,Rh blood type,blood cross matching,direct antiglobulin test,cold antibody absorption test,antibody screening test.Results:(1)Identification of blood type:two methods showed strong agglutination phenomenon can not be interpreted,blood type,but the micro column gel universal cards were incubated again after centrifugation blood results showed clear;(2)Blood cross matching:2 cases of patients with micro column gel universal card shows,primary and secondary side are not agglutinate,result display clear.And Polybrene method shows,main side agglutination,non-agglutination side;(3)Direct antiglobulin test and antibody screening experiments:1 cases of direct antiglobulin test negative,2 cases of direct antiglobulin test positive,2 cases of antibody screening tests were negative;(4)Cold itself antibody absorption test:after absorption of the serum and cell,A,B,O red cells were negative,positive and negative stereotypes line;(5) Cold agglutinin titer of 1:1:256;2 cases a titer of 1:512.Conclusion:The microcolumn gel test has the advantages of simple operation,good repeatability,high sensitivity,convenient for the analysis and identification of cold agglutinin.
